January 27, 2026 Michelle Murray, Authorized Agent Nexus Gerard Family Healing 505 Highway 169 North, Suite 500 Plymouth, MN 55441 RE: Report No. 202510172 Nexus Gerard Family Healing License No. 831080 Dear Michelle Murray: CORRECTION ORDER An investigation of Nexus Gerard Family Healing was completed regarding report number 202510172, which alleged violations of Minnesota Statutes chapter 260E and Minnesota Rules chapter 2960. This maltreatment allegation concerned physical abuse. The disposition of the investigation report is maltreatment not determined. Two violations were noted. VIOLATIONS AND CORRECTION ORDERS The following violations of state and (or) federal laws and rules were observed. Corrective action for each violation is required by Minnesota Statutes, section 245A.06, and is hereby ordered by the Commissioner of Human Services. Failure to correct the violations within the prescribed amount of time may result in fines and/or action against your license, as provided for in Minnesota Statutes, sections 245A.06 and 245A.07. 1. Citation: Minnesota Statutes 245A.66, subdivision 4, paragraph (a) Violation: For five of five staff persons whose records were reviewed (S1-S5), the license holder did not provide training annually on the maltreatment of minors reporting requirements and definitions in chapter 260E. S1-S4 received training on maltreatment of minors in February 2024 and then not again until October 2025. S5 received this training in August 2024 and not again until October 2025. Corrective Action Ordered: On an ongoing basis, you must maintain compliance as required with this subdivision. 2. Citation: Minnesota Rules part 2960.0710, subpart 5 Violation: The license holder did not ensure every physical intervention was documented including the technique used, the time of day, and the name of the staff person and resident involved. On October 24, 2025, a staff person (S5) escorted a resident back to their living unit after they left down a hallway. The incident was not documented as required. Corrective Action Ordered: The facility must retrain S5 on the use of restrictive procedures and the documentation of such, and on an ongoing basis, you must maintain compliance as required with this subdivision. If you believe any of the citations are in error, you may ask the Commissioner of Human Services to reconsider the parts of the correction order that you believe to be in error. If you choose to exercise this right, your request for reconsideration must be in writing and received by the Commissioner within 20 calendar days after receipt of this letter.
