January 30, 2026 PERSONAL SERVICE Michelle Newman, Authorized Agent Pathways to Community 475 Cleveland Avenue North, Suite 100 Saint Paul, Minnesota 55104-5031 License Number 1069743 (HCBS license) License Number 1104285 (CRS license) ORDER OF TEMPORARY IMMEDIATE SUSPENSION Dear Michelle Newman: The Department of Human Services (DHS) is immediately suspending your license to provide home and community-based (HCBS) licensed crisis respite services at the licensed community residential setting (CRS) located at 9515 24th Avenue North, Plymouth, Minnesota (CRS license number 1104285). This immediate suspension is based on a determination that persons served by your program are at an imminent risk of harm. Details of our findings are provided below. Our next steps and your options are also detailed. The immediate suspension goes into effect on February 9, 2026 at 12:01am, or when all service recipients have been transitioned to a new provider, whichever comes first. After this time, you are prohibited from providing home and community-based crisis respite services at the CRS program located at 9515 24th Avenue North, Plymouth, Minnesota (license number 1104285). REASON FOR TEMPORARY IMMEDIATE SUSPENSION There is an imminent risk of harm to persons served. On January 28, 2026, DHS determined that the license holder’s or controlling individual’s actions or failure to comply with applicable law or rule, or the actions of other individuals or conditions in the program, pose an imminent risk of harm to the health, safety, or rights of service recipients receiving crisis respite services at the CRS program located at 9515 24th Avenue North, Plymouth, Minnesota (license number 1104285), based on the following: · Incidents were not responded to or reported as required; · Allegations of suspected maltreatment were not reported as required by mandated reporters; · Allegations of serious maltreatment by neglect in your CRS program located at 9515 24th Avenue North, Plymouth, Minnesota are under investigation. · The license holder did not provide the level of direct service support staff supervision as required in the persons support plans and as required in the federal waiver plan for the service of crisis respite. · Psychotropic medications were administered incorrectly, including a lapse of 4 months. · The license holder did not implement each person’s positive support strategies which resulted in multiple incidents and serious injuries. · Did not implement policies and procedures to ensure the immediate health and welfare of the persons served. Because this report is currently under investigation, no additional information can be released in this order. Based on these findings, DHS cannot ensure the health and safety of the persons served by your program at this time. DHS has determined that the health, safety, and rights of persons in your care are in imminent risk of harm. Therefore, DHS is suspending your license to provide HCBS licensed crisis respite services at CRS license number 1104285. Legal Authority: Minnesota Statutes, section 245A.07, subdivision 2(a)(1). YOUR RIGHT TO APPEAL You have the right to appeal the temporary immediate suspension. Your request must be in writing and clearly state that you are requesting a contested case hearing for this matter. Your request must be made before the deadlines provided below. If you do not meet this deadline, you lose your right to an administrative appeal. Your response time starts when you receive this letter. If you are mailing your request, it must be sent by certified mail and postmarked within 5 calendar days from when you receive this order.
