CORRECTION ORDER
An investigation of Bar None Residential Services was completed regarding report number 202504033 which alleged violation of Minnesota Statutes, section 260E, and Minnesota Rules, parts 2960.0010 to 2960.0120 (CRF). This maltreatment allegation concerned neglect. The disposition of the investigation report is maltreatment not determined. One violation was noted.
VIOLATIONS AND CORRECTION ORDERS
The following violation of state and (or) federal laws and rules was observed. Corrective action for the violation is required by Minnesota Statutes, section 245A.06, and is hereby ordered by the Commissioner of Human Services. Failure to correct the violation within the prescribed amount of time may result in fines and/or action against your license, as provided for in Minnesota Statutes, sections 245A.06 and 245A.07.
Citation: Minnesota Statutes, section 260E.06, subdivision 1.
Violation: A person who knows or has reason to believe a child is being maltreated shall immediately report the information. “Immediately” means as soon as possible but in no event longer than 24 hours.
During the course of the investigation, it was determined that staff persons at multiple levels of authority were aware on May 3, 2025, of possible maltreatment that occurred on April 29, 2025. The Department of Human Services was not aware of the report until May 14, 2025.
Corrective Action Ordered: Immediately and on an ongoing basis, you must maintain compliance with the requirements of Minnesota Statutes and report any incident of suspected maltreatment immediately or within 24 hours.
If you believe any of the citations are in error, you may ask the Commissioner of Human Services to reconsider the parts of the correction order that you believe to be in error. If you choose to exercise this right, your request for reconsideration must be in writing and received by the Commissioner within 20 calendar days after receipt of this letter.
