Dear Carol Acosta:

The Minnesota Department of Children, Youth, and Families (DCYF), Division of Licensing, received your request for reconsideration on December 23, 2025, regarding Citations 2, 3 and 7 in the Amended Correction Order (enclosed) issued to you on December 4, 2025.

On July 29, 2025, DCYF issued a correction order to your program. You requested reconsideration of all eight citations in that correction order. On November 4, 2025, DCYF issued a reconsideration decision affirming Citations 1, 2, 3, 4, 5, 6 and 8, and rescinding Citation 7. DCYF then issued the Amended Correction order on December 4, 2025, in order to reflect the final agency decision.

The Amended Correction Order contains seven citations. All of these citations were previously affirmed in the previous reconsideration decision issued to you on November 4, 2025. Under Minnesota Statutes, section 142B.16, subdivision 2(b), the commissioner’s disposition of a request for reconsideration is final. The reconsideration decision issued to you on November 4, 2025, was a final agency decision as it relates to the seven citations. As such, the citations in the Amended Correction Order are not subject to further reconsideration.1

Previously Ordered Corrective Action. In the original Correction Order, you were ordered to correct the violation(s) cited, and submit documentation of such action to your licensor. A request for reconsideration does not stay any provisions or requirements of the correction order. Minnesota Statutes, 142B.16, subdivision 2, paragraph (a). If you have not done so

already, please immediately comply with the previously ordered corrective actions for all uncontested and affirmed violations.
