CORRECTION ORDER
Dear Dan:
On May 24, June 2, 17, and 20, and August 16, 2025, DHS Licensing received 8 critical incident reports from your facility, 180 Degrees Inc, located at . As a result, DHS is issuing this order which requires you to take the correction action as described below. Details of our findings, next steps, and your options are explained below.
LICENSING VIOLATIONS
DHS determined that your program failed to follow licensing rules and statutes, as described below.
Program Practices
1. Violation: The license holder did not meet critical incident reporting requirements. The following incidents met the definition of a critical incident and were not submitted to DHS within 10 days of learning of the incident, they were submitted late:
a. An incident dated May 11, 2025, was submitted on May 24, 2025;
b. An incident dated May 19, 2025, was submitted on June 2, 2025;
c. Two incidents dated May 20, 2025, were submitted on June 20, 2025;
d. An incident dated May 29, 2025, was submitted on June 17, 2025;
e. An incident dated June 6, 2025, was submitted on June 17, 2025;
f. An incident dated August 2, 2025, was submitted on August 16, 2025; and
g. An incident dated August 3, 2025, was submitted on August 16, 2025.
Rule Violated: Minnesota Rules, parts 2960.0080, subpart 17, item A, and 2960.0020, subpart 24.
Corrective Action Required: Immediately and on an ongoing basis, the license holder must ensure the reporting of critical incidents meets all applicable requirements.

· List each violation you are challenging and identify what is inaccurate or incomplete about the information in the order
· Supply information that is accurate or more complete
