March 24, 2026 Abdirashid Ahmed, Authorized Agent Metro Service Center Inc 2625 East Franklin Avenue Suite LL3 Minneapolis, Minnesota 55406 License Number: 1112794 (245D – Home and Community-Based Services) CORRECTION ORDER Dear Abdirashid Ahmed, On February 12, 2026, a licensing review of Metro Service Center Inc, located at 2625 East Franklin Avenue Minneapolis, Minnesota, was conducted to determine compliance with state and federal laws and rules governing the provision of home and community-based services to persons with disabilities and age 65 and older under Minnesota Statutes, Chapter 245D. As a result of this licensing review a Correction Order is being issued. A. Reason for Correction Order Pursuant to Minnesota Statutes, section 245A.06, if the Commissioner of the Department of Human Services (DHS) finds that the license holder has failed to comply with an applicable law or rule and this failure does not imminently endanger the health, safety, or rights of the persons served by the program, the Commissioner may issue a Correction Order to the license holder. The following violation(s) of state or federal laws and rules were determined as a result of the licensing review. Corrective action for each violation is required by Minnesota Statutes, section 245A.06 and is hereby ordered by the Commissioner of Human Services. 1. Citation: Minnesota Rules, part 9544.0030, subpart 1. Violation: For two of three persons whose record were reviewed (P1 and P2), the license holder did not evaluate positive support strategies as required. The license holder did not evaluate P1’s or P2’s identified positive support strategies with P1 and P2 at least every six months. Corrective Action Ordered: On an ongoing basis, you must maintain compliance as required in this subdivision. 2. Citation: Minnesota Statutes, section 245A.65, subdivision 1. Violation: For two persons whose record were reviewed (P1 and P2), the license holder did not provide orientation to the policies and procedures related to suspected or alleged maltreatment as required. The license holder did not provide P1 and P2 with orientation to the internal and external reporting procedures of alleged or suspected maltreatment of vulnerable adults within 24 hours of admission. The license holder admitted P1 to the program on January 4, 2024, and provided orientation to the internal and external reporting procedures on January 11, 2024. The license holder admitted P2 to the program on February 1, 2024, and provided orientation to the internal and external reporting procedures on February 7, 2024. Corrective Action Ordered: On an ongoing basis, you must maintain compliance as required in this subdivision. 3. Citation: Minnesota Statutes, section 245A.65, subdivision 2. Violation: For two persons whose record were reviewed (P1 and P2), the license holder did not develop and review individual abuse prevention plans (IAPP) as required. a. The license holder did not develop an IAPP for P1 prior to or upon service initiation. The license holder initiated services with P1 on January 4, 2024, and developed P1’s IAPP on January 11, 2024. b. The license holder did not develop an IAPP for P2 prior to or upon service initiation and review the IAPP annually with P2’s interdisciplinary team. The license holder initiated services with P2 on February 1, 2024, developed P1’s IAPP on February 7, 2024, and reviewed P2’s IAPP on October 14, 2024 and November 19, 2025. Corrective Action Ordered: On an ongoing basis, you must maintain compliance as required in this subdivision. 4. Citation: Minnesota Statutes, section 245D.04, subdivision 1. Violation: For one person whose record was reviewed (P1), the license holder did not provide service recipient rights as required. Minnesota Statutes, section 245A.02, subdivision 2b defines “annual” or “annually” to mean prior to or within the same month of the subsequent calendar year. The license holder did not provide P1 with a written notice that identified the service recipient rights and explanation of those rights annually in 2025. The license holder provided P1 with a notice and explanation of the service recipient rights in January 2024 and November 2025. Corrective Action Ordered: On an ongoing basis, you must maintain compliance as required in this subdivision. 5. Citation: Minnesota Statutes, section 245D.07, subdivision 2. Violation: For one person whose record was reviewed (P1), the license holder did not review and revise the preliminary support plan addendum as required. The license holder did not review and revise P1’s preliminary support plan addendum within 60 days of initiating P1’s individualized home supports with training and night supervision services. The license holder developed P1’s preliminary support plan on January 11,2024, and completed the review on March 28, 2024. Corrective Action Ordered: On an ongoing basis, you must maintain compliance as required in this subdivision. 6. Citation: Minnesota Statutes, section 245D.10, subdivision 4. Violation: For two persons whose record were reviewed (P1 and P2), the license holder did not inform and provide policies and procedures as required. Within five working days of service initiation, the license holder did not inform and provide P1’s and P2’s case manager with copies of the following policies and procedures: · grievance policy and procedure; · service suspension and termination policy and procedure; · emergency use of manual restraints policy and procedure; and · data privacy requirements. P1’s services were initiated on January 4, 2024, and P1’s case manager was provided copies of the policies and procedures on January 18, 2024. P2’s services were initiated on February 1, 2024, and February 5, 2024, and P2’s case manager was provided copies of the policies and procedures on February 14, 2024.
Corrective Action Ordered: On an ongoing basis, you must maintain compliance as required in this subdivision. 7. Citation: Minnesota Statutes, section 245D.095, subdivision 3. Violation: For two persons whose record were reviewed (P1 and P2), the license holder did not maintain service recipient records as required. The license holder did not maintain progress or daily log notes for P1 and P2 that were recorded by the program. Corrective Action Ordered: On an ongoing basis, you must maintain compliance as required in this subdivision. 8. Citation: Minnesota Statutes, section 245D.095, subdivision 4. Violation: For two persons whose record were reviewed (P1 and P2), the license holder did not ensure access to service recipient records as required. The license holder did not ensure that all staff providing services to P1 and P2 had access to the persons served information to carry out the support plan or support plan addendum. Corrective Action Ordered: Within 30 days of receiving this order you must ensure staff providing services to P1 and P2 have access to the information in subdivision 1. On an ongoing basis, you must maintain compliance as required in this subdivision. 9. Citation: Minnesota Statutes, section 245D.09, subdivision 5. Violation: For two of two staff persons whose records were reviewed (SP1 and SP2), the license holder did not provide annual staff training as required. "Annual" or "annually" means prior to or within the same month of the subsequent calendar year. a. The license holder did not provide SP1 following trainings annually in 2025: · the safe and correct use of manual restraint on an emergency basis according to the requirements in section 245D.061 and what constitutes the use of restraints, time out, and seclusion, including chemical restraint; and · staff responsibilities related to prohibited procedures under section 245D.06, subdivision 5, why such procedures are not effective for reducing or eliminating symptoms or undesired behavior, and why such procedures are not safe. b. The license holder did not provide SP2 with the following trainings annually in 2026: · the safe and correct use of manual restraint on an emergency basis according to the requirements in section 245D.061 and what constitutes the use of restraints, time out, and seclusion, including chemical restraint; and · staff responsibilities related to prohibited procedures under section 245D.06, subdivision 5, why such procedures are not effective for reducing or eliminating symptoms or undesired behavior, and why such procedures are not safe. Corrective Action Ordered: Within 30 days of receiving this order, you must provide SP1 and SP2 with the annual trainings on the identified above and maintain documentation according to Minnesota Statutes, section 245D.095. On an ongoing basis, you must maintain compliance as required in this subdivision. If you fail to correct the violations specified in the Correction Order within the prescribed time lines the Commissioner may issue an Order of Conditional License or may impose a fine and order other licensing sanctions pursuant to Minnesota Statutes, sections 245A.06 and 245A.07. B. Right to Request Reconsideration If you believe any of the citations are in error, you have the right to request that the Commissioner of Human Services reconsider the parts of the Correction Order that you believe to be in error. The request for reconsideration must be in writing and received by the Commissioner within 20 calendar days after receipt of this report.
