ORDER OF TEMPORARY IMMEDIATE SUSPENSION

Dear Lori Wilson:

The Department of Human Services (DHS) is immediately suspending your license to provide Substance Use Disorder treatment at 1500 Golden Vly Rd, Minneapolis, MN 55411. On March 12, 2026, DHS determined the following health and safety violations:

1. Violation: The license holder did not ensure that the residential treatment program maintained an appropriate license from the Minnesota Department of Health. The Supervised Living Facility license held by the license holder expired on December 31, 2025 and has not been renewed.
