AMENDED CORRECTION ORDER
NOTICE: This Amended Correction Order supersedes a Correction Order issued February 17, 2026, which must be destroyed. The Department of Human Services, Division of Licensing (“DHS”) received your request for reconsideration of the Correction Order on March 2, 2026. This Amended Order reflects the final decision on the request for reconsideration issued on March 19, 2026.
Dear Lewis: On December 8 through 11, 2025, Department of Human Services (DHS) licensors conducted a licensing review and investigation at your facility, Douglas Place Treatment Center LLC dba EOSIS Northern Plains located at 1111 Gateway Dr NE, East Grand Forks, MN 56721. This review was conducted to determine compliance with state and federal laws and rules governing the provision of substance use disorder treatment under Minnesota Statute, chapter 245G. As a result, DHS is issuing this order which requires you to take the correction action as described under each violation. Details of our findings are provided below. Our next steps and your options are also detailed. LICENSING VIOLATIONS
DHS determined that your program failed to follow licensing rules and statutes, as described below. Policies, Procedures, and Practices
1. Violation: The license holder did not meet requirements for receiving public funding reimbursement from the commissioner for services provided as follows:
a. The license holder did not meet requirements for receiving public funding reimbursement from the commissioner for services provided for individuals with co-occurring disorders in accordance with Minnesota Statutes, section 254B.0507, subdivision 6. There was no documentation of a monthly multidisciplinary case review for the months of:
i. April 2025 (client file numbered 5);
ii. May 2025 (client file numbered 16); and
iii. October and November 2025 (client file numbered 2);
b. The license holder did not meet requirements to ensure that all services were provided in the amount and type that were billed for. There was no documentation a daily treatment service was provided on October 19, 2025 for a client receiving ASAM level 3.5 clinically managed high intensity residential treatment (client file numbered 2); and
c. The license holder did not ensure documentation was completed for services provided on October 15 and 21, 2025 (client file numbered 2).
Statute Violated: Minnesota Statutes, section 245A.167, paragraph (a) and 245A.191, paragraph (a). Corrective Action Required: Immediately, and on an ongoing basis, the license holder must ensure and document that services provided meet the requirements for payment from the behavioral health fund. The noncompliance identified above may result in nonpayment of claims submitted by the license holder for public program reimbursement; recovery of payments made for the services; disenrollment in the public payment program; or other administrative, civil, or criminal penalties provided by law. 2. Violation: The license holder did not meet requirements governing service initiation policies. The service initiation policy did not contain service initiation preferences that comply with the Code of Federal Relations, title 45, part 96.131.
Statute Violated: Minnesota Statutes, section 245G.14, subdivision 1. Corrective Action Required: Immediately and on an ongoing basis, the license holder must ensure that the service initiation policy meets all applicable requirements. Within 30 days of receipt of this order, submit a service initiation policy that meets all applicable requirements. 3. Violation: The license holder did not meet requirements governing grievance procedures. The grievance procedure did not meet requirements in the following ways:
a. Six of forty-four grievances reviewed did not have documentation that the license holder responded to client’s grievance within three days of a staff member’s receipt of the grievance on the following dates:
1) July 26, 2025;
2) August 24, 2025;
3) September 16, 2025;
4) October 18, 2025: and
5) November 14 and 15, 2025.
Statute Violated: Minnesota Statutes, section 245G.15, subdivision 2, clause (3). Corrective Action Required: Immediately and on an ongoing basis, the license holder must ensure that the grievance procedure meets all applicable requirements.
4. Violation: The license holder did not meet requirements governing health services for residential treatment. The written procedures for assessing and monitoring a client’s health were not signed by a registered nurse.
Statute Violated: Minnesota Statutes, section 245G.21, subdivision 7. Corrective Action Required: Immediately and on an ongoing basis, the license holder must ensure that the health services policy for residential treatment meets all applicable requirements. Within 30 days of receipt of this order, submit written procedures for assessing and monitoring a client’s health that meets all applicable requirements.
5. Violation: The license holder did not meet requirements governing behavioral emergency procedures. The procedure did not include:
a. Contact information for emergency resources that staff must consult when a client’s behavior cannot be controlled by the behavior emergency procedures; and
b. Circumstances under which behavioral emergency procedures may be used.
Statute Violated: Minnesota Statutes, section 245G.16, paragraph (a), clauses (2) and (4). Corrective Action Required: Immediately and on an ongoing basis, the license holder must ensure that behavioral emergency procedures meet all applicable requirements. Within 30 days of receipt of this order, submit a behavioral emergency procedure that meets all applicable requirements.
6. Violation: The license holder did not meet requirements governing maltreatment of vulnerable adults. An internal review dated November 15, 2025 did not contain an evaluation of whether there is a need for any corrective action to be taken by the license holder to protect the health and safety of vulnerable adults.
Statute Violated: Minnesota Statutes, section 245A.65, subdivision 1, paragraph (b), clause (1). Corrective Action Required: Immediately and on an ongoing basis, the license holder must ensure that vulnerable adult internal reviews meet all applicable requirements.
Personnel Files
7. Violation: One of two personnel files reviewed for requirements governing staff qualifications for individuals with temporary permit (personnel file numbered 9) did not meet requirements. There was no documentation of the type of supervision provided for the following dates:
a. May 23 and 26, 2025;
b. June 6, 20, and 26, 2025;
c. July 15, 17, and 24, 2025; and
d. August 1, 2025.
Statute Violated: Minnesota Statutes, section 245G.11, subdivision 11, paragraph (a). Corrective Action Required: Immediately and on an ongoing basis, the license holder must ensure that supervision documentation meets all applicable requirements.
Client Files
8. Violation: One of five client files reviewed for requirements governing client orientation (client file numbered 5) did not meet requirements. There was no documentation that the license holder sought consent to disclose suspected maltreatment upon admission to the facility.
Statute Violated: Minnesota Statutes, section 245A.65, subdivision 1a, paragraph (b). Corrective Action Required: Immediately and on an ongoing basis, the license holder must ensure that maltreatment consent meets all applicable requirements. 9. Violation: One of five client files reviewed for requirements governing initial services plans (ISP) (client file numbered 2) did not meet requirements. The ISP was not completed within 24 hours of the day of service initiation.
Statute Violated: Minnesota Statutes, section 245G.04, subdivision 1. Corrective Action Required: Immediately and on an ongoing basis, the license holder must ensure that initial service plans meet all applicable requirements. 10. Violation: One of five client files reviewed for requirements governing comprehensive assessments (CA) (client file numbered 5) did not meet requirements. The CA did not contain a description of the client’s symptoms.
Statute Violated: Minnesota Statutes, section 245G.05, subdivision 3. Corrective Action Required: Immediately and on an ongoing basis, the license holder must ensure that comprehensive assessments meet all applicable requirements. 11. Violation: Four of five client files reviewed for requirements governing individual treatment plans (ITP) did not meet requirements in the following ways:
a. The ITP was not completed within ten days from the day of service initiation (client file numbered 1);
b. The ITP did not include how the family will be involved in the client’s treatment (client file numbered 3); and
c. The ITP did not include a treatment strategy (client files numbered 3, 4, and 5).
Statute Violated: Minnesota Statutes, section 245G.06, subdivision 1 and 1a, clause (3). Corrective Action Required: Immediately and on an ongoing basis, the license holder must ensure that individual treatment plans meet all applicable requirements. 12. Violation: Five of five client files reviewed for requirements governing client record documentation did not meet requirements in the following ways:
a. Documentation was not completed within seven days of provision of the treatment service on April 21, 2025 (client file numbered 5); and
b. Documentation of concerns related to attendance for treatment services, including the reason for any client absence from a treatment service was not completed on the date that each occurs for the following dates:
i. April 25 and May 12, 16, and 18, 2025 (client file numbered 5);
ii. May 12 and 13, 2025 (client file numbered 4);
iii. October 12, 14, 16 17, 20, and 23, 2025 (client file numbered 2);
iv. November 12 and 13, 2025 (client file numbered 3); and
v. November 18, 21, 25, and 27 (client file numbered 1).
Statute Violated: Minnesota Statutes, section 245G.06, subdivision 2a. Corrective Action Required: Immediately and on an ongoing basis, the license holder must ensure that documentation of treatment services meet all applicable requirements. 13. Violation: One of three client files reviewed for requirements governing treatment plan reviews (TPR) (client file numbered 2) did not meet requirements. A treatment plan review was not completed every 14 days for a client receiving ASAM level 3.5 high intensity residential services. The treatment plan review was due on October 9, 2025; however, was not completed until October 15, 2025.
Statute Violated: Minnesota Statutes, section 245G.06, subdivision 3a, paragraph (b). Corrective Action Required: Immediately and on an ongoing basis, the license holder must ensure that treatment plan reviews meet all applicable requirements. 14. Violation: Four of five client files reviewed for requirements governing medication administration did not meet requirements. The client’s use of medication was not recorded, including staff signatures with date and time on the following dates:
a. April 19 and May 9, 11, 13, 17, 18, and 20, 2025 (client file numbered 5);
b. August 7 and 8, 2025 (client file numbered 6);
c. November 1, 2025 (client file numbered 2); and
d. October 28 and November 4, 8, 11, 13, 16, and 17, 2025 (client file numbered 3);
Statute Violated: Minnesota Statutes, sections 245G.08, subdivisions 5, paragraph (c), clause (7) and 6, clause (3) and 245G.21, subdivision 8. Corrective Action Required: Immediately and on an ongoing basis, the license holder must ensure that meet all applicable requirements. 15. Violation: Two of three client files reviewed for requirements governing discharge summaries did not meet requirements in the following ways:
a. The services discharge summary was not completed within five days of the client’s service termination (client file numbered 3); and
b. The discharge summary did not include the client’s progress towards achieving each goal identified in the individual treatment plan (client file numbered 4).
Statute Violated: Minnesota Statutes, section 245G.06, subdivision 4, paragraphs (a) and (b), clause (2). Repeat Violation: The license holder was cited for a similar violation in the Correction order dated April 03, 2025. Corrective Action Required: Immediately and on an ongoing basis, the license holder must ensure that discharge summaries meet all applicable requirements. 16. Violation: Ten of fifteen client files (clients numbered 3 and 7 through 15) reviewed for requirements governing client property management did not meet requirements. The license holder did not immediately document receipt and disbursement of the person’s funds or other property at time of receipt or disbursement.
Statute Violated: Minnesota Statutes, section 245A.04, subdivision 13, paragraph (c), clause (1). Corrective Action Required: Immediately and on an ongoing basis, the license holder must ensure that client property management meets all applicable requirements. Written Response Required
If you fail to correct the violation(s) specified in the Correction Order within the prescribed time lines the Commissioner may issue an Order of Conditional License or may impose a fine and order other licensing sanctions pursuant to Minnesota Statutes, sections 245A.06 and 245A.07. As stated above, this amended Correction Order reflects the final agency decision in your request for reconsideration of the Correction Order dated February 17, 2026. As such, the decision is not subject to appeal.
