April 6, 2026 Tim Osmondson 7581 9th Street N Ste 100 Oakdale, MN 55128 RE: Report No. 202600373 Jobs House of Prosperity License No. 1103884/1124584 Dear Tim Osmondson: CORRECTION ORDER An investigation of Jobs House of Prosperity was completed regarding report number 202600373 which alleged violation of Minnesota Statutes, section 626.557, and Minnesota Statutes, Chapter 245D. This maltreatment allegation concerned neglect. The disposition of the investigation report is inconclusive. Four violations were noted. VIOLATIONS AND CORRECTION ORDERS The following violations of state and (or) federal laws and rules were observed. Corrective action for each violation is required by Minnesota Statutes, section 245A.06, and is hereby ordered by the Commissioner of Human Services. Failure to correct the violations within the prescribed amount of time may result in fines and/or action against your license, as provided for in Minnesota Statutes, sections 245A.06 and 245A.07. 1. Citation: Minnesota Statutes, section 245D.095, subdivision 5. Violation: The license holder did not maintain personnel records for a staff person (P1) as required. A signed copy of P1’s training on the C’s plans was not maintained in P1’s personnel records. Corrective Action Ordered: On an ongoing basis, you must maintain compliance as required in this subdivision. 2. Citation: Minnesota statutes section 245A.65, subdivision 3. Violation: The license holder did not provide a staff person (P2) orientation to vulnerable adult reporting requirements and all policies and procedures related to the prevention and reporting of maltreatment within 72 hours of first providing direct contact services to a person. On October 3, 2024, P2 first provided direct support to a client (C). The license holder did not provide P2 orientation to vulnerable adult reporting requirements until February 3, 2026. Correction Action Ordered: On an ongoing basis, you must maintain compliance as required in this subdivision. 3. Citation: Minnesota statutes section 626.557, subdivision 3, paragraph (a). Violation: More than one mandated reporter who had reason to believe that a vulnerable adult was or had been maltreated did not immediately report the information to the common entry point. On January 11, 2026, multiple staff persons were aware of possible neglect, but a report was not made to the common entry point until over 24 hours after the incident, on January 12, 2026. Corrective Action Ordered: Correct immediately and maintain compliance with this licensing requirement on a continuing basis throughout the program. 4. Citation: Minnesota statutes section 245D.06, subdivision 1, paragraph (b). Violation: The license holder must report incidents to the person's case manager within 24 hours of an incident occurring while services were being provided. On January 11, 2026, the C left the facility without supervision, and a notification was not made to the C’s case manager until January 12, 2026, over 24 hours after the incident. Corrective Action Ordered: On an ongoing basis, you must maintain compliance as required in this subdivision. If you believe any of the citations are in error, you may ask the Commissioner of Human Services to reconsider the parts of the correction order that you believe to be in error. If you choose to exercise this right, your request for reconsideration must be in writing and received by the Commissioner within 20 calendar days after receipt of this letter.
