April 7, 2026 Theresa Bluhm 12805 Highway 55 STE 400 Minneapolis, Minnesota 55441 Report Number: 202511004 License Number(s): 1069019/1069015 CORRECTION ORDER Dear Theresa Bluhm: An investigation of Homeward Bound Inc was completed regarding report number 202511004which alleged violation of Minnesota Statutes, section 626.557, and Minnesota Statutes, Chapter 245D. This maltreatment allegation concerned neglect. The disposition of the investigation report is an error. Two violations were noted. A. Reason for Correction Order Pursuant to Minnesota Statutes, section 245A.06, if the Commissioner of the Department of Human Services (DHS) finds that the license holder has failed to comply with an applicable law or rule and this failure does not imminently endanger the health, safety, or rights of the persons served by the program, the Commissioner may issue a Correction Order to the license holder. The following violations of state or federal laws and rules were determined. Corrective action for the violations is required by Minnesota Statutes, section 245A.06 and is hereby ordered by the Commissioner of Human Services. 1. Citation: Minnesota Statutes, section 245D.05, subdivision 1, paragraph (a) and (b). Violation: For one facility person (C) whose record was reviewed, the license holder failed to provide services in response to C’s identified health needs as specified in the support plan and the support plan addendum and did not maintain documentation on how C’s needs will be met. C’s Health Needs Record identified provider instructions that two staff persons assist with all cares or one staff person acknowledged and thanked C reminding him/her to relax while the second staff person completed the cares. The facility did not have further documentation clarifying the provider’s instructions. Corrective Action Ordered: Comply with 245D requirements related to the provision of services in accordance with the person’s support plan and support plan addendum. 2. Citation: Minnesota Statutes, section 245A.04, subdivision 14, (b), (1-3), states that “The license holder shall: (1) provide training to program staff related to their duties in implementing the program's policies and procedures developed under paragraph (a); (2) document the provision of this training; and (3) monitor implementation of policies and procedures by program staff.” Violation: The facility’s Bathing Guidelines Procedure stated that staff persons, “do not assist individuals with a bath or shower with only one staff in the house”. Information from three staff people at the facility was consistent that on multiple occasions, the C and other facility person’s received bathing/showering assistance from only one staff person. Corrective Action Ordered: Comply with Minnesota Statutes, section 245A.04, subdivision 14 requirements related to monitoring the implementation of policies and procedures by program staff.” If you do not correct the violation specified in the Correction Order within the prescribed time lines the Commissioner may issue an Order of Conditional License or may impose a fine and order other licensing sanctions pursuant to Minnesota Statutes, sections 245A.06 and 245A.07. B. Right to Request Reconsideration If you believe the citation is in error, you have the right to request that the Commissioner of Human Services reconsider the parts of the Correction Order that you believe to be in error. The request for reconsideration must be in writing and received by the Commissioner within 20 calendar days after receipt of this report.
