CORRECTION ORDER

Dear Ms. Ricchio:

On February 26, 2026, a Department of Children, Youth, and Families (DCYF) licensor conducted a licensing review at your facility located at 8441 Wayzata Blvd, Suite 220, Golden Valley, MN. As a result of this visit, DCYF determined that you are in violation of one of the child placing agency rules and statutes. As a result, DCYF is issuing this order which requires you to take the correction action as described below. Details of our findings are provided below. Our next steps and your options are also detailed.

LICENSING VIOLATIONS

DCYF determined that your program failed to follow licensing rules and statutes, as described below.

Adoption Files

1. Violations: Adoption files were incomplete as follows:

i. Adoption file 1: The individual fact sheet was not completed on the current version of the form.

ii. Adoption file 2: The individual fact sheet was not completed on the current version of the form. A home study update was completed instead of an original home study assessment. The last home study update expired 1/26/19 and the new update was completed on 6/5/24 which is over five-year allowable timeframe.

iii. Adoption file 3: The individual fact sheet was not completed on the current version of the form.

iv. Adoption file 5: On the application, not all areas were addressed. One applicant failed to include information in all areas on the individual fact sheet. The home study update completed on 7/19/23 also required the signature of the county supervisor due to this being a shared document.

v. Adoption file 7: The individual fact sheet was not completed on the current version of the form. The application and individual fact sheets dated 12/26/24 did not have a corresponding home study update as required.

vi. Adoption file 8: The individual fact sheet was not completed on the current version of the form. One applicant’s individual fact sheet was missing the name of the applicant who was completing the form. Background studies were completed on 1/12/24 and 3/11/24 which was prior to receiving the signed application on 3/24/24.

vii. Adoption file 9: The original and update applications were not filled out completely. One applicant did not answer question six on the individual fact sheet. One applicant noted information on the individual fact sheet, but it was not included in the home study assessment.

viii. Adoption file 10: A new application was not completed for the annual update. One applicant’s individual fact sheet was not completely filled out and not signed and dated.

ix. Adoption file 11: One applicant did not answer question six on the individual fact sheet.

x. Adoption file 12: The individual fact sheet was not completed on the current version of the form. Background studies were completed on 8/31/25 which was prior to receiving the signed application on 10/29/25.

Rule Violated: Minnesota Rules, part 9545.0835, subpart 2.

Additional applicable Statute violation for Adoption file 5: Minnesota Statutes, 142B.30, subdivision 5.

Repeat Violation: In a Correction Order that DCYF issued your agency on 5/10/23, you were previously found in violation of this same Rule when one adoption file was missing the name of one of the applicants on the individual fact sheet.

Corrective action required

You must correct the violations above. Submit documentation to your licensor within 30 days from when you received this order explaining the corrective action taken to ensure ongoing compliance.

YOUR RIGHT TO REQUEST RECONSIDERATION

You have the right to request reconsideration of this order in writing. Your request must:

1. Specify the parts of the correction order that are alleged to be in error;

2. Explain why they are in error; and

3. Include documentation to support the allegation of error.

If you are mailing your request, it must be sent by certified mail and postmarked within 20 calendar days from when you received this order. If you do not meet this deadline, you lose your right to request reconsideration. The timeline to appeal began when you received this order. Please send it to:

Legal authority for this licensing action

· This action is taken under Minnesota Statutes, section 142B.16, subdivision 1.

· This child placing agency must maintain compliance with the licensing statutes and rules, and specifically Minnesota Rules, 9545.0755 through 9545.0845; Minnesota Statute, section 142B.10, subdivision 18; and Minnesota Statute, section 259.37, subdivision 2.

· The timeline to request reconsideration of the order is provided in Minnesota Statutes, section 142B.16, subdivision 2.
