Terms of Settlement Agreement:
1. License Holder’s family child care license, License No. 1096227, will be placed on conditional status for a period of two (2) years from the effective date of the Settlement Agreement. While under the conditional license, License Holder will comply with the following terms:
a. License Holder will follow and comply with all applicable Minnesota Statutes and Rules, including Minnesota Rules, parts 9502.0300 through 9502.0445, and background study requirements under Minnesota Statutes, chapter 245C.
b. License Holder’s family child care license will be limited to a maximum capacity of 12 children (class C2 license), and no variance to age distribution of capacity will be granted during the conditional period.
c. License Holder must submit all outstanding licensing documents to Steele County within calendar 30 days of the effective date of the Agreement. The documents include: training records from January 1, 2023 to December 31, 2025, completed child enrollment forms, current records for pediatric first aid and CPR trainings, current provider policy, fire and storm drill log for 2025, crib inspection forms for 2025, and others deemed outstanding by the County.
d. License Holder must work with Steele County to obtain a mentor to work on ensuring that she is following all family child care rules and laws. The mentor must be obtained within 30 calendar days of the effective date of the Agreement, and must be approved by Steele County. Upon completion of working with the mentor License Holder must submit a summary to Steele County of what she learned and changes she will implement into her program to support required compliance with licensing regulations including record keeping and paperwork.
e. License Holder must post the Settlement Agreement in her family child care program in a conspicuous place and provide a copy of this Agreement to parents or guardians of all children in care under License No. 1096227. She must obtain signatures from parents or guardians of each child attending her family child care programs indicating they have received a copy of the Agreement. Documentation that the parents received a copy of the Agreement must be provided to Steele County within 30 calendar days of the effective date of the Agreement. For new families enrolling a child in the family child care program, License Holder must submit documentation signed by the parent or guardian to Steele County within five (5) calendar days of the child beginning to receive family child care services from her family child care program that verifies that the parent received a copy of the Agreement.
