May 4, 2026 Hamdi Omar, Authorized Agent United Providers LLC 4001 Stinson Boulevard NE Suite 140 Saint Anthony, Minnesota 55421 License Number: 1115996 (245D – HCBS) License Number: 1118758 (CRS) CORRECTION ORDER Dear Hamdi Omar: On April 1, 2026, a licensing review of United Providers LLC located at 4001 Stinson Boulevard, Suite 140, Saint Anthony, Minnesota, was conducted to determine compliance with state and federal laws and rules governing the provision of home and community-based services to persons with disabilities and age 65 and older under Minnesota Statutes, Chapter 245D. As a result of this licensing review a Correction Order is being issued. A. Reason for Correction Order Pursuant to Minnesota Statutes, section 245A.06, if the Commissioner of the Department of Human Services (DHS) finds that the license holder has failed to comply with an applicable law or rule and this failure does not imminently endanger the health, safety, or rights of the persons served by the program, the Commissioner may issue a Correction Order to the license holder. The following violation(s) of state or federal laws and rules were determined as a result of the licensing review. Corrective action for each violation is required by Minnesota Statutes, section 245A.06 and is hereby ordered by the Commissioner of Human Services. 1. Citation: Minnesota Statutes, section 245A.65, subdivision 2. Violation: For one of two persons whose record was reviewed (P1), the license holder did not meet the requirements for program abuse prevention plans as required. The license holder did not post a current copy of the program abuse prevention plan in a prominent location in the program, CRS license number 1118758, and be available upon request to mandated reporters, persons receiving services, and legal representatives. The PAPP posted at the program did not identify the use of cameras and the license holder stated that the PAPP posted was outdated. Corrective Action Ordered: Within 30 days of receiving this order, you must post the current PAPP in a prominent location in the program. On an ongoing basis, you must maintain compliance as required in this subdivision. 2. Citation: Minnesota Statutes, section 245D.071, subdivision 5, paragraph (g). Violation: For one person whose record was reviewed (P1), the license holder did not meet service plan review and evaluation as required. P1’s support plan addendum required the license holder to send progress review reports summarizing the person’s status and progress toward achieving identified outcomes at least five days prior to the progress review meeting. P1’s progress review meeting was held on April 25, 2025. The license holder did not provide P1’s progress review report until April 25, 2025. Corrective Action Ordered: On an ongoing basis, you must maintain compliance as required in this subdivision. 3. Citation: Minnesota Statutes, section 245D.07, subdivision 1 Violation: For one person whose record was reviewed (P1) the license holder did not comply with the requirements of the federal waiver plan. The license holder maintained monitoring equipment throughout the program, CRS license number 1118758 where P1 resided and received services. The license holder did not receive approval for the use of monitoring technology from the lead agency.
Corrective Action Ordered: Within 30 days of receiving this order, you must either remove or receive consent for all monitoring technology being used at the program site. If you choose to receive consent and continue the use of monitoring technology, use of the monitoring technology must meet the requirements of the federal waiver plan. The lead agency approves and oversees the use of all monitoring technology so you must consult with the case managers for P1. Additionally, the same consultation must occur for all service recipient receiving services where monitoring technology is being utilized. You must inform your licensor, in writing, of your decision and submit completed DHS-6789B or DHS-6789C forms for all service recipients. On an ongoing basis, you must maintain compliance as required in this subdivision. 4. Citation: Minnesota Statutes, section 245D.07, subdivision 1a. Violation: For one person whose record was reviewed (P1), the license holder did not provide services in response to the person’s identified needs, interests, preferences, and desired outcomes as required. The license holder identified one of P1’s outcomes as “P1 will complete some form of exercise, either by walking or following a video exercise on YouTube and will use his/her phone’s Pacer app to track his/her steps, up to 3 times per week, with no more than 1 verbal prompt for 75% of trials over 9 of 12 months.” This outcome was implemented on April 25, 2025. The license holder did not collect data on P1’s outcome from April 25, 2025, to the time of the licensing review. Corrective Action Ordered: Within 30 days of receiving this order, you must begin collecting data on P1’s outcome. On an ongoing basis, you must maintain compliance as required in this subdivision. 5. Citation: Minnesota Statutes, sections 245D.05, subdivision 2 and 245D.095, subdivision 3, paragraph (b). Violation: For one person whose record was reviewed (P1), the license holder did not follow medication administration procedures and maintain the service recipient record as required. During a site visit completed during the licensing review on April 1, 2026, DHS licensors learned that the license holder administered an over the counter medication for P1 without a prescription and was not transcribed on P1’s MAR. The license holder did not maintain current orders for P1’s medication and did not implement medication administration procedures for P1 to ensure P1 took medications as prescribed. Corrective Action Ordered: Within 30 days of receiving this order, you must obtain and document the prescriber’s current written or electronically recorded order or prescription for P1’s medication that includes the person’s name, description of the medication to be provided, and the frequency and other information needed to safely and correctly administer the medication to ensure effectiveness. On an ongoing basis, you must maintain compliance as required in this subdivision. 6. Citation: Minnesota Statutes, section 245D.05, subdivision 4. Violation: For one person whose record was reviewed (P2), the license holder did not review and report medication and treatment issues as required. The license holder did not report to P2’s refusal or failure to take or receive medication or treatment as prescribed, to P2’s case manager as they occurred. The license holder was assigned responsibility for P2’s medication administration and maintained documentation that P2’s medications were not administered over 100 times from October 2025 through March 2026. Corrective Action Ordered: Within 30 days of receiving this order you must: · complete a review of P2’s medication administration records from October 2025 to present; · notify P2’s case manager when P2’s medications were not administered as prescribed; and · maintain documentation of this notification in P2’s record. On an ongoing basis, you must maintain compliance as required in this subdivision. 7. Citation: Minnesota Statutes, section 245D.051, subdivision 1. Violation: For one person whose record was reviewed (P1), the license holder did not meet the conditions of psychotropic medication administration as required. P1 received multiple psychotropic medications. The license holder did not maintain documentation for P1 that included a description of the target symptoms that each psychotropic medication was to alleviate. Corrective Action Ordered: Within 30 days of receiving this order, you must maintain documentation in P1’s record that includes a description of that target symptoms that each psychotropic medication is prescribed to alleviate. On an ongoing basis, you must maintain compliance as required in this subdivision. 8. Citation: Minnesota Statutes, section 245D.09, subdivision 5. Violation: For two of three staff persons whose record were reviewed (SP1 and SP2), the license holder did not provide annual training as required. Minnesota Statutes, section 245A.02, subdivision 2b defines “annual” or “annually” to mean prior to or within the same month of the subsequent calendar year a. The license holder did not provide SP1 with the following annual training in 2025: · the principles of person-centered service planning and delivery as identified in section 245D.07, subdivision 1a, and how they apply to direct support service provided by the staff person; · the program abuse prevention plan according to the requirements in 245A.65, subdivision 3. b. The license holder did not provide SP2 with annual training on the principles of person-centered service planning and delivery as identified in section 245D.07, subdivision 1a, and how they apply to direct support services provided by the staff person annually in 2026.
Corrective Action Ordered: Within 30 days of receiving this order, you must provide SP1 and SP2 with the above mentioned training and maintain documentation of this training in their personnel records. On an ongoing basis, you must maintain compliance as required in this subdivision. 9. Citation: Minnesota Statutes, section 245D.095, subdivision 5. Violation: For one staff person whose record was reviewed (SP2), the license holder did not maintain personnel records as required. The license holder did not maintain documentation of SP2’s orientation and training in SP2’s personnel record including: · the date the training was completed, · the number of hours per subject area, and · the name of the trainer or instructor. Corrective Action Ordered: On an ongoing basis, you must maintain compliance as required in this subdivision. If you fail to correct the violations specified in the Correction Order within the prescribed time lines the Commissioner may issue an Order of Conditional License or may impose a fine and order other licensing sanctions pursuant to Minnesota Statutes, sections 245A.06 and 245A.07. B. Right to Request Reconsideration If you believe any of the citations are in error, you have the right to request that the Commissioner of Human Services reconsider the parts of the Correction Order that you believe to be in error. The request for reconsideration must be in writing and received by the Commissioner within 20 calendar days after receipt of this report.
