May 20, 2026 Fadumo Mohamed Osman, Authorized Agent New Found Hope LLC 750 Second Street Northeast, Suite 132 Hopkins, Minnesota 55343 License Number: 1109873 CORRECTION ORDER Dear Fadumo Mohamed Osman: On April 28, 2026, a licensing review of New Found Hope LLC, located at 750 Second Street Northeast, Suite 132, Hopkins, Minnesota, was conducted to determine compliance with state and federal laws and rules governing the provision of home and community-based services to persons with disabilities and age 65 and older under Minnesota Statutes, Chapter 245D. As a result of this licensing review a Correction Order is being issued. A. Reason for Correction Order Pursuant to Minnesota Statutes, section 245A.06, if the Commissioner of the Department of Human Services (DHS) finds that the license holder has failed to comply with an applicable law or rule and this failure does not imminently endanger the health, safety, or rights of the persons served by the program, the Commissioner may issue a Correction Order to the license holder. The following violation(s) of state or federal laws and rules were determined as a result of the licensing review. Corrective action for each violation is required by Minnesota Statutes, section 245A.06 and is hereby ordered by the Commissioner of Human Services. 1. Citation: Minnesota Statutes, section 245D.05, subdivision 1. Violation: For one of two persons whose records were reviewed (P1), the license holder did not maintain documentation of health service needs as required. The license holder was responsible for meeting P1’s health service needs, including the administration of an injectable medication in emergency situations. The license holder did not maintain documentation on how P1’s health service needs would be met, including a description of the procedures the license holder would follow in order to provide medication administration and monitor health conditions according to written instructions from a licensed health professional. Corrective Action Ordered: Within 30 days of receiving this order, you must maintain documentation on how P1’s health service needs will be met, including a description of the procedures the license holder will follow in order to provide medication administration and monitor health conditions according to written instructions from a licensed health professional. On an ongoing basis, you must maintain compliance as required in this subdivision. 2. Citation: Minnesota Statutes, section 245D.07, subdivision 2. Violation: For one person whose record was reviewed (P2), the license holder did not meet service planning and delivery for basic support services as required. The license holder did not review and revise P2’s preliminary support plan addendum to document the services that would be provided including how, when, and by whom services would be provided, and the person responsible for overseeing the delivery and coordination of services within 60 calendar days of service initiation. Corrective Action Ordered: Within 30 days of receiving this order, you must review and revise P2’s preliminary support plan addendum to document the services that will be provided including how, when, and by whom services will be provided, and the person responsible for overseeing the delivery and coordination of services. On an ongoing basis, you must maintain compliance as required in this subdivision. 3. Citation: Minnesota Statutes, section 245D.071, subdivision 4. Violation: For one person whose record was reviewed (P1), the license holder did not develop a service plan that documented service outcomes and supports as required. The license holder did not develop a service plan for P1 that documented the service outcomes and supports based on the assessments and person centered planning within ten working days of P1’s initial planning meeting. The license holder held P1’s initial service planning meeting on February 19, 2024, and developed supports and methods for P1’s outcomes on January 2, 2025. Corrective Action Ordered: On an ongoing basis, you must maintain compliance as required in this subdivision. 4. Citation: Minnesota Statutes, section 245D.071, subdivision 5. Violation: For one person whose record was reviewed (P1), the license holder did not conduct service plan review and evaluation as required. The license holder did not summarize P1’s status and progress toward achieving the identified outcomes, make recommendations and identify the rationalization for continuing, or discontinuing implementation of supports and methods identified in subdivision 4 in a report available at the time of P1’s progress review meeting on January 4, 2026. Corrective Action Ordered: On an ongoing basis, you must maintain compliance as required in this subdivision. 5. Citation: Minnesota Statutes, Minnesota Statutes, section 245D.10, subdivision 4. Violation: For one person whose record was reviewed (P1), the license holder did not provide written or electronic copies of policies and procedures as required. The license holder did not inform P1’s case manager of the policies and procedures and provide copies of the following policies and procedures, within 5 working days of service initiation. • grievance policy; • temporary service suspension policy; • service termination policy; • emergency use of manual restraints policy; and • data privacy policy. Corrective Action Ordered: Within 30 days of receiving this order, you must provide P1’s case manager with the policies and procedures documented above. On an ongoing basis, you must maintain compliance as required in this subdivision. 6. Citation: Minnesota Rule, 9544.0030, subpart 1. Violation: For two persons whose records were reviewed (P1 and P2), the license holder did not evaluate positive support strategies as required. The license holder did not evaluate the identified positive support strategies with P1 and P2 every 6 months. Corrective Action Ordered: Within 30 days of receiving this order, you must evaluate P1’s and P2’s identified positive support strategies. Based upon the results of the evaluation, you must determine whether changes are needed in the positive support strategies used, and, if so, make appropriate changes. On an ongoing basis, you must maintain compliance as required in this subdivision. 7. Citation: Minnesota Statutes, section 245A.65, subdivision 3. Violation: For two of three staff persons whose records were reviewed (SP1 and SP2), the license holder did not provide orientation to the program abuse prevention plan (PAPP) as required. a. The license holder did not provide SP1 with an orientation to the PAPP within 72 hours of first providing direct contact services. At the time of the review, the license holder had not provided SP1 with an orientation to the PAPP. b. The license holder did not provide annual training to SP2 in 2024 and 2025 on the PAPP. Corrective Action Ordered: Within 30 days of receiving this order, you must provide SP1 and SP2 with training to the PAPP and document this orientation in their personnel records. On an ongoing basis, you must maintain compliance as required in this subdivision. 8. Citation: Minnesota Statutes, section 245D.09, subdivision 4. Violation: For two staff persons whose records were reviewed (SP1 and SP2), the license holder did not provide and ensure completion of orientation as required. a. The license holder did not provide SP1 with the following orientation sufficient to create staff competency for direct support staff: · the job description and how to complete specific job functions, including: o responding to and reporting incidents as required under section 245D.06, subdivision 1; and o following safety practices established by the license holder and as required in section 245D.06, subdivision 2; and · the license holder’s current policies and procedures required under this chapter, including their location and access, and staff responsibilities related to implantation of those policies and procedures. b. The license holder did not provide SP1 and SP2 with training to individual needs. SP1 and SP2 work with an individual that used an injectable medication. The license holder did not document training to SP1 and SP2 on the proper use of this medication. Corrective Action Ordered: Within 30 days of receiving this order, you must provide the orientation to SP1 in the areas listed above and maintain documentation of this training in SP1’s personnel record. You must provide training to SP1 and SP2 on the use of injectable medications and document this training in their personnel records. Training on injectable medications must be provided in accordance with Minnesota Statutes, section 245D.05, subdivision 5. On an ongoing basis, you must maintain compliance as required in this subdivision. 9. Citation: Minnesota Statutes, section 245D.09, subdivision 5. Violation: For three staff persons whose records were reviewed (SP1-SP3), the license holder did not provide annual training as required. Minnesota Statutes, section 245A.02, subdivision 2b defines “annual” or “annually” to mean prior to or within the same month of the subsequent calendar year. a. The license holder did not provide annual training to SP1 in 2025 in the following areas: · data privacy requirements; · the service recipient rights and staff responsibilities related to ensuring the exercise and protection of those rights according to the requirements in Minnesota Statutes, section 245D.04; · sections 245A.65, 245A.66, and 626.557 and chapter 260E, governing maltreatment reporting and service planning for children and vulnerable adults, and staff responsibilities related to protecting persons from maltreatment and reporting maltreatment; · the principles of person-centered service planning and delivery as identified in Minnesota Statutes, section 245D.07, subdivision 1a, and how they applied to direct support service provided by the staff person; · the safe and correct use of manual restraint on an emergency basis according to the requirements in section 245D.061 and what constitutes the use of restraints , time out, and seclusion, including chemical restraint; · staff responsibilities related to prohibited procedures under section 245D.06, subdivision 5, why such procedures are not effective for reducing or eliminating symptoms or undesired behavior, and why such procedures are not safe; and · strategies to minimize the risk of sexual violence, including concepts of healthy relationships, consent, and bodily autonomy of people with disabilities. SP1 completed orientation training in February 2024 and annual training in November 2025. b. The license holder did not provide annual training to SP2 and SP3 in 2024 and 2025 in the following areas: · data privacy requirements; · the service recipient rights and staff responsibilities related to ensuring the exercise and protection of those rights according to the requirements in Minnesota Statutes, section 245D.04; · sections 245A.65, 245A.66, and 626.557 and chapter 260E, governing maltreatment reporting and service planning for children and vulnerable adults, and staff responsibilities related to protecting persons from maltreatment and reporting maltreatment; · the principles of person-centered service planning and delivery as identified in Minnesota Statutes, section 245D.07, subdivision 1a, and how they applied to direct support service provided by the staff person; · the safe and correct use of manual restraint on an emergency basis according to the requirements in section 245D.061 and what constitutes the use of restraints , time out, and seclusion, including chemical restraint; · staff responsibilities related to prohibited procedures under section 245D.06, subdivision 5, why such procedures are not effective for reducing or eliminating symptoms or undesired behavior, and why such procedures are not safe; and · strategies to minimize the risk of sexual violence, including concepts of healthy relationships, consent, and bodily autonomy of people with disabilities. SP2 completed the trainings above on February 26, 2026. SP3 completed the trainings above on January 1, 2026. c. The license holder did not provide basic first aid training to SP1 and SP2 in 2025. Corrective Action Ordered: Within 30 days of receiving this order, you must provide SP1 and SP2 with basic first aid training. On an ongoing basis, you must maintain compliance as required in this subdivision. 10. Citation: Minnesota Statutes, section 245D.095, subdivision 5. Violation: For one staff person whose record was reviewed (SP1), the license holder did not maintain personnel records as required. The license holder did not document the number of hours per subject area for SP1’s orientation training. Corrective Action Ordered: On an ongoing basis, you must maintain compliance as required in this subdivision. If you fail to correct the violations specified in the Correction Order within the prescribed time lines the Commissioner may issue an Order of Conditional License or may impose a fine and order other licensing sanctions pursuant to Minnesota Statutes, sections 245A.06 and 245A.07. B. Right to Request Reconsideration If you believe any of the citations are in error, you have the right to request that the Commissioner of Human Services reconsider the parts of the Correction Order that you believe to be in error. The request for reconsideration must be in writing and received by the Commissioner within 20 calendar days after receipt of this report.
