May 21, 2026 Joanna Etzler, Authorized Agent Valley Lake Boy’s Home, Inc. 3850 200th Avenue Breckenridge, MN 56520 License Number: 1036970 (CRF) CORRECTION ORDER Dear Joanna Etzler: On March 2, 2026, a Department of Human Services (DHS) licensor conducted a certification review at your facility, Valley Lake Boys Home, Inc. located at 3850 200th Avenue, Breckenridge, MN 56520. This review was conducted to determine compliance with state and federal laws and rules governing the provision of The Family First Prevention Services Act under Minnesota Statutes, section 245A.25. As a result, DHS is issuing this order which requires you to take the correction action as described under each violation. Details of our findings are provided below. Our next steps and your options are also detailed. LICENSING VIOLATIONS DHS determined that your program did not follow licensing rules and statutes, as described below. Personnel Files 1. Violation: Four of four files reviewed for the requirements governing orientation (personnel files (PF) numbered 1, 2, 3 and 4) did not meet requirements. The files did not contain documentation the staff was trained on the following topics prior to direct contact: a. Concepts of trauma-informed care and how to provide services to each youth according to those concepts; b. Impacts of each youth’s culture, race, gender, and sexual orientation on the youth’s behavioral health and traumatic experiences; and c. The human trafficking training approved by the Department of Children, Youth and Family Services Administration (DCYF). Statute Violated: Minnesota Statutes, section 245A.25, subdivisions 3, paragraph (h) and subdivision 5, paragraph (f). Repeat Violation: In a Correction Order that DHS issued on July 12, 2023, you were previously found in violation of this same Statute. Corrective Action Required: Immediately and on an ongoing basis, the license holder must ensure orientation training meets all applicable requirements. Within 30 days of receipt of this order, submit documentation that staff (PF numbered 1, 2, 3 and 4) completed the required training listed in paragraph (a) through (c) above. 2. Violation: Two of two files reviewed for the requirements governing on-going training did not meet requirements. The files did not contain documentation the staff were trained annually on the following: a. Concepts of trauma informed care and how to provide services according to these concepts in calendar year (CY) 2025 (PF numbered 4) and CY 2026 (2PF); b. Impacts of race, gender and sexual orientation on individual youth’s behavioral health and experiences of trauma in CY 2025 (PF numbered 4); c. Impacts of culture, race, gender and sexual orientation on individual youth’s behavioral health and experiences of trauma in CY 2026 (PF numbered 2); and d. Human trafficking training approved by DCYF CY 2025 (PF numbered 4) and CY2026 (PF numbered 4). Statute Violated: Minnesota Statutes, section 245A.25, subdivisions 3, paragraph (h) and 5, paragraph (f). Repeat Violation: In a Correction Order that DHS issued on July 12, 2023, you were previously found in violation of this same Statute. Corrective Action Required: Immediately and on an ongoing basis, the license holder must ensure ongoing training meets all applicable requirements. Within 30 days of receipt of this order, submit documentation that staff (PF 2 and 4) completed the training required in paragraphs (a) through (d) above. Resident Files 3. Violation: Two of two resident files reviewed for the requirements governing program services (resident files (RF) numbered 1 and 3) did not meet requirements. The files did not contain documentation showing engagement of the youth and their family with the discharge planning process. Statute Violated: Minnesota Statute 245A.25, subdivision 5, paragraph (d), clause 6. Repeat Violation: In a Correction Order that DHS issued on July 12, 2023, you were previously found in violation of this same Statute. Corrective Action Required: Immediately and on an ongoing basis, the license holder must ensure ongoing training meets all applicable requirements. Within 30 days of receipt of this order, submit documentation that demonstrates compliance. Written Response Required If you fail to correct the violation(s) specified in the Correction Order within the prescribed time lines the Commissioner may issue an Order of Conditional License or may impose a fine and order other licensing sanctions pursuant to Minnesota Statutes, sections 245A.06 and 245A.07.
