June 1, 2026 Timothy Osmondson 7581 9th St N suite 100 Oakdale, MN 55128 RE: Report No. 202603104 Jobs House of Prosperity License No. 1103884 Dear Timothy Osmondson: CORRECTION ORDER An investigation of Jobs House of Prosperity was completed regarding report number 202603104 which alleged violation of Minnesota Statutes, section 260E, and Minnesota Statutes, Chapter 245D. This maltreatment allegation concerned physical abuse. The disposition of the investigation report is maltreatment not determined. Four violations were noted. VIOLATIONS AND CORRECTION ORDERS The following violations of state and (or) federal laws and rules were observed. Corrective action for each violation is required by Minnesota Statutes, section 245A.06, and is hereby ordered by the Commissioner of Human Services. Failure to correct the violations within the prescribed amount of time may result in fines and/or action against your license, as provided for in Minnesota Statutes, sections 245A.06 and 245A.07. 1. Citation: Minnesota Statutes, section 245D.06, subdivision 1 (b). Violation: The license holder did not complete incident response and reporting as required. The C was involved in incidents of emergency use of manual restraint on March 14, 25, and 31, 2026. The license holder did not report the incidents to the C’s guardian or case manager within 24 hours of the incidents occurring while services were provided. The license holder reported the above incidents to the C’s guardian and case manager on March 16, 27, and April 24, 2026, respectively. Corrective Action Ordered: On an ongoing basis, you must maintain compliance as required in this subdivision. 2. Citation: Minnesota Statutes, section 245D.061. Violation: The license holder did not ensure that emergency use of manual restraints complied with the requirements of this chapter. The C was involved in incidents involving the emergency use of manual restraints on March 14, 27, 30, 31, and April 10 and 15, 2026. The license holder did not: · complete the internal reviews within five working days of the emergency use of manual restraints for the March 31, 2026 incident. The license holder completed this on April 24, 2026; · submit the following to the Department of Human Services and the Office of the Ombudsman for Mental Health and Developmental Disabilities within five working days of the expanded support team review for the instances of emergency use of manual restraint which occurred on March 14, 27, 30, 31, and April 10 and 15, 2026: o the report required under subdivision 5; o the internal review and corrective action plan required under subdivision 6; and o the summary of the expanded support team review required under subdivision 7. Corrective Action Ordered: Within 30 days of receiving this order, you must complete the above mentioned items for the C and maintain documentation of what was completed in the C’s service recipient record. On an ongoing basis, you must maintain compliance as required in this subdivision. 3. Citation: Minnesota Statutes, section 245D.61, subdivision 7, paragraphs (a) and (b). Violation: Within five working days of the completion of the internal review of each restraint, the license holder consulted with the expanded support team and maintained a copy of the support team discussion. Two incidents involving the emergency use of a manual restraint occurred on March 31, 2026, and the expanded support team was not notified and completed their review until April 24, 2026. Corrective Action Ordered: On an ongoing basis, you must provide training as required in this subdivision. 4. Citation: Minnesota Statutes, section 245A.65, subdivision 3. Violation: For one of one staff person whose record was reviewed (P1), the license holder did not provide annual training as required. The license holder did not provide mandated reporter training to P1 annually. This training was provided to P1 on June 13, 2024, and was not completed again until May 19, 2026. Corrective Action Ordered: On an ongoing basis, you must provide training as required in this subdivision. If you believe any of the citations are in error, you may ask the Commissioner of Human Services to reconsider the parts of the correction order that you believe to be in error. If you choose to exercise this right, your request for reconsideration must be in writing and received by the Commissioner within 20 calendar days after receipt of this letter.
