ORDER OF LICENSE REVOCATION

Dear Fahad Mahdi:

The Department of Children, Youth, and Families (DCYF) is revoking your license to provide child care services at 2940 Chicago Avenue, Minneapolis, Minnesota. This revocation is based on a notice DCYF received from the Minnesota Department of Revenue (DOR) that you have failed to pay a tax or government debt as required by state law. Details of our findings are provided below. Our next steps and your options are also detailed.

This revocation goes into effect June 2, 2026, at 9:00pm.

REASON FOR LICENSE REVOCATION

1.  Failure to Pay Tax or Government Debt

Pursuant to Minnesota Statutes, section 270C.72, subdivision 1, if DCYF receives a notice from DOR that an applicant, licensee or certificate holder owes the state delinquent taxes, penalties, or interest, or has failed to file tax returns, DCYF shall revoke or refuse to issue, transfer, or renew the license as directed by DOR.

On April 28, 2026, the Department of Children, Youth, and Families (DCYF) notified you that DCYF had received formal notice from the Minnesota Department of Revenue (DOR) that you have failed to pay a tax or government debt as required by state law. In that letter, DCYF also informed you that DCYF is required to revoke your license within 30 days unless DCYF receives a clearance certificate from DOR. DOR previously sent you a letter detailing why it intends to revoke your license.

As of the date of this letter, DCYF has not received a tax clearance certificate from DOR.

Therefore, pursuant to Minnesota Statutes, section 270C.72, subdivision 1, your license is revoked.

Please review the enclosed notice from DOR on how to make a payment.

Legal authority for this notice

· This notice is sent in accordance with Minnesota Statutes, section 270C.72, subdivision 1(c), which requires the licensing authority to revoke the license 30 days after receiving the notice from DOR, unless it receives a tax clearance from DOR.
