June 24, 2026 Leanne Negley, Authorized Agent Rural Living Environments Inc 105 Central Blvd Babbitt, MN 55706 Report Number: 202509940 License Numbers: 1076682/1068090 CORRECTION ORDER Dear Leanne: An investigation of Rural Living Environments Inc was completed regarding report number 202509940 which alleged violation of Minnesota Statutes, section 626.557, and Minnesota Statutes, Chapter 245D. This maltreatment allegation concerned financial exploitation. The disposition of the investigation report is substantiated. Two violations were noted. A. Reason for Correction Order Pursuant to Minnesota Statutes, section 245A.06, if the Commissioner of the Department of Human Services (DHS) finds that the license holder has failed to comply with an applicable law or rule and this failure does not imminently endanger the health, safety, or rights of the persons served by the program, the Commissioner may issue a Correction Order to the license holder. The following violation of state or federal laws and rules was determined. Corrective action for the violation is required by Minnesota Statutes, section 245A.06 and is hereby ordered by the Commissioner of Human Services. 1. Citation: Minnesota Statutes, section 245A.04, subdivision 13, paragraph (c), clause 1. Violation: The license holder did not ensure safekeeping of a consumer’s (C’s) funds by immediately documenting receipt and disbursement of the C’s funds at the time of receipt or disbursement, including providing a signature of the person or payee. Corrective Action Ordered: On an ongoing basis, you must maintain compliance as required in this subdivision. 2. Citation: Minnesota Statutes, section 245D.095, subdivision 5. Violation: Four staff persons (SP1, SP2, SP3, and SP4) whose records were reviewed the license holder did not maintain personnel records as required. As part of this investigation of alleged maltreatment, a DHS investigator requested copies of SP1’s-SP4’s personnel records that included documentation of SP1’s-SP4’s training on the C’s individual service recipient needs/plans. The license holder was unable to provide documentation to the DHS investigator. Corrective Action Ordered: On an ongoing basis, you must maintain compliance as required in this subdivision for required training and maintaining documentation of the completed training in personnel records. If you do not correct the violation specified in the Correction Order within the prescribed time lines the Commissioner may issue an Order of Conditional License or may impose a fine and order other licensing sanctions pursuant to Minnesota Statutes, sections 245A.06 and 245A.07. B. Right to Request Reconsideration If you believe the citation is in error, you have the right to request that the Commissioner of Human Services reconsider the parts of the Correction Order that you believe to be in error. The request for reconsideration must be in writing and received by the Commissioner within 20 calendar days after receipt of this report.
