CORRECTION ORDER Dear William Lockett: On June 8, 9, and 12, 2026, a Department of Human Services (DHS) licensor conducted a licensing review at your facility, Micah Halfway House (The) located at 3047 5th Avenue South, Minneapolis, Minnesota 55403. This review was conducted to determine compliance with state and federal laws and rules governing the provision of substance use disorder treatment services under Minnesota Statute, chapter 245G. As a result, DHS is issuing this order which requires you to take the correction action as described under each violation. Details of our findings are provided below. Our next steps and your options are also detailed. LICENSING VIOLATIONS DHS determined that your program failed to follow licensing rules and statutes, as described below. Client Files 1. Violation: The license holder did not meet requirements to ensure that all services were provided in the amount and type that were billed for. There was no documentation a daily treatment service was provided for a client receiving ASAM level 3.5 clinically managed high intensity residential treatment for the following dates: a. May 30 and 31, 2026 (client files numbered 1 and 2); and b. May 24, 25, 30, and 31, 2026 (client file numbered 3). Statute Violated: Minnesota Statutes, section 245A.167, paragraph (a). Corrective Action Required: Immediately, and on an ongoing basis, the license holder must ensure and document that services are provided in the amount for which they are billed. The noncompliance identified above may result in nonpayment of claims submitted by the license holder for public program reimbursement; recovery of payments made for the services; disenrollment in the public payment program; or other administrative, civil, or criminal penalties provided by law. 2. Violation: Two of three client files reviewed for requirements governing comprehensive assessments (client files numbered 1 and 3) did not meet requirements. There was no documentation that a staff member reviewed and updated a previously received comprehensive assessment by the end of the fifth day in which a treatment service was provided. Statute Violated: Minnesota Statutes, section 245G.05, subdivision 1, paragraph (c). Repeat Violation: The license holder was previously cited for similar violations in the following orders: · Correction Order dated February 20, 2026; · Correction Order dated April 28, 2025; and · Order of Conditional License dated July 17, 2024 Corrective Action Required: Immediately, and on an ongoing basis, the license holder must ensure that comprehensive assessments meet all applicable requirements. Within 30 days of receipt of this order, submit 1 comprehensive assessment that meets all applicable requirements. 3. Violation: Three of three client files reviewed for requirements governing individual treatment plans (ITP) did not meet requirements. The ITP: a. Was not completed by the end of the tenth day on which a treatment session has been provided from the day of service initiation (client files numbered 1 and 3); and b. Did not document the following: i. A treatment strategy (client files numbered 2 and 3); and ii. A schedule for accomplishing the client’s treatment goals and objectives (client file numbered 2). Statute Violated: Minnesota Statutes, section 245G.06, subdivisions 1 and 1a, paragraph (a). Corrective Action Required: Immediately, and on an ongoing basis, the license holder must ensure that individual treatment plans meet all applicable requirements. Within 30 days of receipt of this order, submit 1 individual treatment plan that meets all requirements. 4. Violation: Three of three client files reviewed for requirements governing client record documentation did not meet requirements in the following ways: a. There was no documentation of a significant event that occurred on May 21, 2026within 24 hours of the event (client file numbered 3); b. Entries in the client record were not accurate. Documentation of treatment services contained conflicting amounts of service provided on the following dates: i. May 26, 27, June 1, and 2, 2026 (client file numbered 1); ii. May 26, 27, 29, June 1, and 2, 2026 (client file numbered 2); and iii. May 26 through 29, June 1, and 2, 2026 (client file numbered 3); and c. Documentation of treatment services did not include: i. Type for: 1. May 27, 28, 29, and June 2, 2026 (client file numbered 1); and 2. May 27, 28, 29, and June 1, 2026 (client files numbered 2 and 3); ii. Job title of the staff person making the entry for: 1. May 27, 28, 29, June 1, and 2, 2026 (client file numbered 1); 2. May 26 through 29, June 1, and 2, 2026 (client file numbered 2); and 3. May 21, 26 through 29, June 1, and 2, 2026 (client file numbered 3). Statute Violated: Minnesota Statutes, section 245G.06, subdivisions 2a and 2b. Repeat Violation: The license holder was previously cited for similar violations in the following orders: · Correction Order dated February 20, 2026; · Correction Order dated April 28, 2025; and · Order of Conditional License dated July 17, 2024 Corrective Action Required: Immediately, and on an ongoing basis, the license holder must ensure that client record documentation meets all applicable requirements. Within 30 days of receipt of this order, submit 2 client record documents that meet all applicable requirements. 5. Violation: One of three client files reviewed for requirements governing administration of medications did not meet requirements. The client’s use of medication was not recorded, including staff signatures with date and time on June 6 and 7, 2026 (client file numbered 1). Statute Violated: Minnesota Statutes, section 245G.08, subdivision 6, clause (3) and 245G.21, subdivision 8. Repeat Violation: The license holder was previously cited for similar violations in the following orders: · Correction Order dated February 20, 2026; · Correction Order dated April 28, 2025; and · Order of Conditional License dated July 17, 2024 Corrective Action Required: Immediately, and on an ongoing basis, the license holder must ensure that administration of medication documentation meets all applicable requirements. 6. Violation: One client file reviewed for requirements governing service discharge summaries did not meet requirements (client file numbered 4). The service discharge summary did not document a service termination diagnosis. Statute Violated: Minnesota Statutes, section 245G.06, subdivision 4, paragraph (b). Corrective Action Ordered: Immediately, and on an ongoing basis, the license holder must ensure that service discharge summaries meet all applicable requirements. Written Response Required If you fail to correct the violation(s) specified in the Correction Order within the prescribed time lines the Commissioner may issue an Order of Conditional License or may impose a fine and order other licensing sanctions pursuant to Minnesota Statutes, sections 245A.06 and 245A.07.
