July 27, 2026 Samuel Ongethi, Authorized Agent Pillar Healthcare Services LLC 5701 Shingle Creek Parkway, Suite 375 Brooklyn Center, Minnesota 55430-2384 License Number: 1108728 (245D – HCBS) CORRECTION ORDER Dear Samuel Ongethi: On June 25, 2026, a licensing review of Pillar Healthcare Services LLC, located at 5701 Shingle Creek Parkway, Brooklyn Center, Minnesota, was conducted to determine compliance with state and federal laws and rules governing the provision of home and community-based services to persons with disabilities and age 65 and older under Minnesota Statutes, Chapter 245D. As a result of this licensing review a Correction Order is being issued. A. Reason for Correction Order Pursuant to Minnesota Statutes, section 245A.06, if the Commissioner of the Department of Human Services (DHS) finds that the license holder has failed to comply with an applicable law or rule and this failure does not imminently endanger the health, safety, or rights of the persons served by the program, the Commissioner may issue a Correction Order to the license holder. The following violation(s) of state or federal laws and rules were determined as a result of the licensing review. Corrective action for each violation is required by Minnesota Statutes, section 245A.06 and is hereby ordered by the Commissioner of Human Services. 1. Citation: Minnesota Statutes, section 245A.65, subdivision 1, paragraph (c). Violation: For two of three persons whose records were reviewed (P1 and P2), the license holder did not provide an orientation to the internal and external reporting procedures as required. a. The license holder initiated services for P1 on February 13, 2026. The license holder did not provide P1 with an orientation to the internal and external reporting procedures related to alleged or suspected maltreatment within 24 hours of admission. The license holder provided P1 with this orientation on March 20, 2026. b. The license holder initiated services for P2 to the program on February 12, 2026. The license holder did not provide P2 with an orientation to the internal and external reporting procedures related to alleged or suspected maltreatment within 24 hours of admission. The license holder provided P2 with this orientation on March 26, 2026. Repeat Violation: In a Correction Order that DHS issued on December 16, 2025, you were previously found in violation of this same statute. Corrective Action Ordered: On an ongoing basis, you must maintain compliance as required in this subdivision. 2. Citation: Minnesota Statutes, section 245A.65, subdivision 2, paragraph (b) Violation: For two persons whose records were reviewed (P1 and P2), the license holder did not develop an individualized abuse prevention plan (IAPP) as required. a. The license holder did not develop an individual abuse prevention plan (IAPP) for P1 that included an individualized assessment of P1’s susceptibility to abuse by other individuals, including other vulnerable adults and P1’s risk of abusing other vulnerable adults. b. The license holder did not develop an IAPP for P2 that included an individualized assessment of P2’s susceptibility to abuse by other individuals including other vulnerable adults and P2’s risk of abusing other vulnerable adults. The license holder documented in the IAPP that P2 was not vulnerable to abuse in any areas. This information was not consistent with information documented elsewhere in P2’s plan. Repeat Violation: In a Correction Order that DHS issued on December 16, 2025, you were previously found in violation of this same statute. Corrective Action Ordered: Within 30 days of receiving this order, you must: · conduct an individualized assessment of P1’s susceptibility to abuse by other individuals, including other vulnerable adults and P1’s risk of abusing other vulnerable adults; · develop an IAPP for P1 that includes statements of the specific measures to be taken to minimize the risk of abuse to P1 and other vulnerable adults and the specific actions to be taken to minimize the risk of abuse within the scope of licensed services that are based on the individualized assessment of P1; · provide the IAPP to P1 and P1’s case manager for review; · conduct an individualized assessment of P2’s susceptibility to abuse by other individuals, including other vulnerable adults, and P2’s risk of abusing other vulnerable adults; · develop an IAPP for P2 that includes statements of the specific measures to be taken to minimize the risk of abuse to P2 and other vulnerable adults and the specific actions to be taken to minimize the risk of abuse within the scope of licensed services that are based on the individualized assessment of P2; · provide the IAPP to P2 and P2’s case manager for review; and · provide training to all staff that work with P1 and P2 on the updated IAPP and maintain documentation of this training in the personnel record including the date the training is completed, the number of hours per subject area, and the name of the trainer or instructor. On an ongoing basis, you must maintain compliance as required in this subdivision. 3. Citation: Minnesota Statutes, section 245D.05, subdivision 1. Violation: For one person whose record was reviewed (P1), the license holder did not maintain documentation on health needs as required. The license holder maintained documentation that they would assist P1 with health management and symptom monitoring. The license holder did not maintain documentation on how P1’s health needs would be met, including a description of the procedures the license holder would follow in order to: · provide medication assistance according to this chapter; and · monitor health conditions according to written instructions from a licensed health professional. Corrective Action Ordered: Within 30 days of receiving this order, you must: · document in P1’s support plan addendum how P1’s health needs will be met including the procedures to be followed in order to provide medication assistance and monitor health conditions according to written instructions from a licensed health professional; and · provide training to all staff that work with P1 on the updated support plan addendum and maintain documentation of this training in the personnel record including the date the training is completed, the number of hours per subject area, and the name of the trainer or instructor. On an ongoing basis, you must maintain compliance as required in this subdivision. 4. Citation: Minnesota Statutes, section 245D.10, subdivision 4. Violation: For two persons whose records were reviewed (P1 and P2), the license holder did not make policies and procedures available as required. a. The license holder initiated services for P1 on February 13, 2026. The license holder did not inform and provide copies of the policies and procedures affecting P1’s rights to P1 and P1’s case manager within five days of service initiation. The license holder provided the policies and procedures to P1 and P1’s case manager on March 20, 2026. b. The license holder initiated services for P2 on February 16, 2026. The license holder did not inform and provide copies of the policies and procedures affecting P2’s rights to P2 and P2’s case manager withing five days of service initiation. The license holder provided the policies and procedures to P2 and P2’s case manager on March 26, 2026. Repeat Violation: In a Correction Order that DHS issued on December 16, 2025, you were previously found in violation of this same statute. Corrective Action Ordered: On an ongoing basis, you must maintain compliance as required in this subdivision. 5. Citation: Minnesota Statutes, section 245D.09, subdivision 4. Violation: For one of three persons whose records were reviewed (SP3), the license holder did not provide orientation to the program as required. The license holder did not provide orientation to program requirements within 60 days of hire in the following areas: · the principles of person-centered service planning and delivery as identified in section 245D.07, subdivision 1a, and how they apply to direct support service provided by the staff person; · staff responsibilities related to prohibited procedures under section 245D.06, subdivision 5, or successor provisions, why such procedures are not effective for reducing or eliminating symptoms or undesired behavior, and why such procedures are not safe; and · strategies to minimize the risk of sexual violence, including concepts of healthy relationships, consent, and bodily autonomy of people with disabilities. Corrective Action Ordered: Within 30 days of receiving this order, you must provide SP3 with orientation in the areas outlined above and maintain documentation of this training in the personnel record, including the date the training is completed, the number of hours per subject area, and the name of the trainer or instructor. On an ongoing basis, you must maintain compliance as required in this subdivision. 6. Citation: Minnesota Statutes, section 245D.09, subdivision 5. Violation: For one staff person whose record was reviewed (SP1), the license holder did not provide annual training as required. The license holder did not ensure SP1 completed annual training in 2026 in the following areas: · the principles of person-centered service planning and delivery as identified in section 245D.07, subdivision 1a, and how they apply to direct support service provided by the staff person; · staff responsibilities related to prohibited procedures under section 245D.06, subdivision 5, or successor provisions, why such procedures are not effective for reducing or eliminating symptoms or undesired behavior, and why such procedures are not safe; and · strategies to minimize the risk of sexual violence, including concepts of healthy relationships, consent, and bodily autonomy of people with disabilities. SP1 last completed training in this area in May 2025. Repeat Violation: In a Correction Order that DHS issued on December 16, 2025, you were previously found in violation of this same statute. Corrective Action Ordered: Within 30 days of receiving this order, you must provide SP1 with annual training in the areas outlined above. Documentation of this training must be maintained in the personnel record including the date the training is completed, the number of hours per subject area, and the name of the trainer or instructor. On an ongoing basis, you must maintain compliance as required in this subdivision. 7. Citation: Minnesota Statutes, section 245D.081, subdivision 3. Violation: The license holder did not ensure program management and oversight occurred as required. The license holder identified SP4 as the staff person responsible for the duties assigned to the designated manager. Those duties included ensuring that corrective action was taken when ordered by the commissioner. In an order dated December 16, 2025, the license holder was ordered to provide the policies and procedures affecting a service recipients rights to the case manager for P3 within 30 days of receiving the order. The license holder provided the policies and procedures to P3’s case manager on March 13, 2026, which was past the deadline for the corrective action ordered. The license holder did not ensure SP4 provided program management and oversight of the services provided. Corrective Action Ordered: On an ongoing basis, you must maintain compliance as required in this subdivision. If you fail to correct the violations specified in the Correction Order within the prescribed time lines the Commissioner may issue an Order of Conditional License or may impose a fine and order other licensing sanctions pursuant to Minnesota Statutes, sections 245A.06 and 245A.07. B. Right to Request Reconsideration If you believe any of the citations are in error, you have the right to request that the Commissioner of Human Services reconsider the parts of the Correction Order that you believe to be in error. The request for reconsideration must be in writing and received by the Commissioner within 20 calendar days after receipt of this report.
