August 13, 2026 Anab Abdi, Authorized Agent Sprout and Bloom Child Development Center 4015 Brookdale Cir N Brooklyn Park, MN 55443 License Number: 2001704 (Child Care Ctr) Program Location: 15 121st Ave NW, Coon Rapids, MN 55448 Dear Anab Abdi: The Minnesota Department of Children, Youth, and Families (DCYF), Division of Licensing, received your request for reconsideration regarding Citation 1 in the Correction Order issued to you on February 12, 2026. You did not request review for Citations 2, 3, 4, 5, 6, 7, 8, 9, and 10. Citations 2, 3, 4, 5, 6, 7, 8, 9, and 10 are not under the scope of review. Reconsideration Determination Citation 1 Violation. Staff distribution requirements were not maintained. Applicable Law. The license holder must ensure that the following requirements for staff distribution are met and a written staff distribution record is kept in the administrative record. A. Only a staff person who is qualified as a teacher, assistant teacher, or aide and who works directly with children can be counted in meeting the staff-to-child ratios. B. An assistant teacher may be substituted for a teacher during morning arrival and afternoon departure times if the total arrival and departure time does not exceed 25 percent of the center's daily hours of operation. C. The maximum group size applies at all times except during meals, outdoor activities, field trips, naps and rest, and special activities such as films, guest speakers, and holiday programs. D. Except as provided in item B, staff distribution within each age category must follow the pattern in subitems (1) to (4). (1) The first staff member needed to meet the required staff-to-child ratio must be a teacher. (2) The second staff member must have at least the qualifications of a child care aide. (3) The third staff member must have at least the qualifications of an assistant teacher. (4) The fourth staff member must have at least the qualifications of a child care aide. The pattern in subitems (1) to (4) must be repeated until the number of staff needed to meet the staff-to-child ratio for each age category has been achieved. Minnesota Rules, part 9503.0040, subpart 2. Your Response. You stated, “On the day of the visit, 9 toddlers and 4 preschoolers were present; one preschooler had left for an appointment. When another arrived, the total was 14. Staff will ensure real-time counts are clearly communicated and documented when combining classrooms.” Reconsideration Determination. The Licensor observed that toddlers and preschoolers were combined in the large motor room. The Licensor asked the Toddler class room staff how many toddlers were present, and the Toddler staff person indicated that there were nine toddlers. The Licensor also asked the Preschool class room staff person how many preschool children were present, and the Preschool class room staff person stated there were five preschoolers. The Licensor counted the total number of children and noted that there were 14 children total. Later during the site visit, the Licensor observed that a preschool-aged child arrived approximately at 10:46am. The preschool-aged child was brought into the large motor room with the other 14 children. The total number of children in the large motor room was 15. There was one teacher and two aides supervising the 15 children. You were required to have an assistant teacher qualified staff person in addition to the teacher and aide to meet staff distribution requirements. Citation 1 is affirmed because staff distribution was not met. Disposition The Commissioner has reviewed the relevant laws and all the information you submitted in response to the Correction Order. Citation 1 is affirmed. You did not request review for Citations 2, 3, 4, 5, 6, 7, 8, 9, 10. Citations 2, 3, 4, 5, 6, 7, 8, 9 and 10 are not under the scope of review. This is a final agency decision. Previously Ordered Corrective Action. In the original Correction Order issued, you were ordered to correct the violation(s) cited, and submit documentation of such action to your licensor. A request for reconsideration does not stay any provisions or requirements of the correction order. Minnesota Statutes, 142B.16, subdivision 1, paragraph (a). If you have not done so already, please immediately comply with the previously ordered corrective actions for all uncontested and affirmed violations and respond in the Provider Hub immediately.
