August 20,2026 Michelle Murray, Authorized Agent 505 Highway 169 North, Suite 500 Plymouth, MN 55441 RE: Report No. 202511813 Nexus Gerard Family Healing License No. 831080 Dear Michelle Murray: CORRECTION ORDER An investigation of Nexus Gerard Family Healing was completed regarding report number 202511813 which alleged violation of Minnesota Statutes, section 260E, and Minnesota Rules, parts 2960.0010 to 2960.0120 (CRF). This maltreatment allegation concerned Neglect. The disposition of the investigation report is maltreatment determined. One violation was noted. VIOLATIONS AND CORRECTION ORDERS The following violation of state and (or) federal laws and rules was observed. Corrective action for the violation is required by Minnesota Statutes, section 245A.06, and is hereby ordered by the Commissioner of Human Services. Failure to correct the violations within the prescribed amount of time may result in fines and/or action against your license, as provided for in Minnesota Statutes, sections 245A.06 and 245A.07. 1. Citation: Minnesota Rules Part 2960.0050, subpart 1, item R Violation: The license holder did not ensure a clients (C) right to be free from restraint or seclusion, used for a purpose other than to protect the C from imminent danger to self or others. On December 18, 2025, a staff person (S) attempted the use of restrictive procedure on the C, which resulted in injury, when the C was not in imminent danger or a threat to self or others. Corrective Action Ordered: The facility must ensure all staff persons are retrained on the use of restrictive procedures, and on an ongoing basis, must maintain compliance as required within this subdivision. If you believe any of the citations are in error, you may ask the Commissioner of Human Services to reconsider the parts of the correction order that you believe to be in error. If you choose to exercise this right, your request for reconsideration must be in writing and received by the Commissioner within 20 calendar days after receipt of this letter.
