August 19, 2026 Jody Miller, Authorized Agent Children’s Home & LSS, LLC dba Children’s Home 1605 Eustis Street St. Paul, MN 55108 License Number: 1127565 Child Placing Agency Report Numbers: 202601108(CH) and 202601104 (LSS) CORRECTION ORDER Dear Ms. Miller: On February 3, 2026, the Department of Children, Youth, and Families (DCYF) Child Foster Care Licensing Unit received a complaint regarding your agency’s application and licensing processes. As a result of the investigation, DCYF determined that you are in violation of one licensing statute. As a result, DCYF is issuing this order which requires you to take corrective action as described below. Our next steps and your options are also detailed. LICENSING VIOLATIONS DCYF determined that your program failed to follow licensing statute, as described below: 1. Violation: Children’s Home failed to provide applicants with an application prescribed by the commissioner when they provided prospective foster care applicants with a Children’s Home & LSS (CHLSS) application rather than the Minnesota Adoption and Child Foster Care Application, the application form prescribed by the commissioner. Additionally, the agency proceeded with assessing child foster care applicants when they requested releases of information, gathered collateral information, and, at times, requested additional assessments prior to receipt of the Commissioner’s application form. Statute Violated: Minnesota Statutes, section 142B.10, subdivision 1, paragraph (a). While the licensing investigation began under both license numbers (Children’s Home Society of MN and Lutheran Social Service of MN), due to the merger, Children’s Home is now the child placing agency responsible. Corrective action required You must correct the violation cited above. Submit documentation to DCYF CFC Licensing Consultant, Taylor Johnson, within 30 days of when you receive this order explaining how you have corrected the violations. · Children’s Home must discontinue assessing and evaluating prospective child foster care applicants prior to receiving a signed Commissioner’s CFC Application. · Additionally, you must cease using the Children’s Home Application Part 1 form and only use the Commissioner’s CFC Application. · Children’s Home must submit a written plan on how you will ensure that all prospective child foster care applicants will receive a Commissioner’s CFC Application immediately. · Children’s Home must submit a list of any current applicants that you are working with that do not have a Commissioner’s CFC Application or Preliminary Information Form (PIF) submitted to DCYF. · Children’s Home must submit a written plan on how you will ensure that all current applicants complete a Commissioner’s CFC Application. Additionally, the plan must articulate how you will ensure a PIF is submitted to DCYF Licensing as soon as possible, but no later than 7 business days after receiving a signed Commissioner’s CFC Application. YOUR RIGHT TO REQUEST RECONSIDERATION You have the right to request reconsideration of this order in writing. Your request must: 1. Specify the parts of the correction order that are alleged to be in error; 2. Explain why they are in error; and 3. Include documentation to support the allegation of error. Legal authority for this licensing action · This action is taken under Minnesota Statutes, section 142B.16, subdivision 1. · This child placing agency must maintain compliance with the licensing statutes and rules, specifically Minnesota Statutes, Chapter 142B, and Minnesota Statutes, Chapter 245C, and Minnesota Rules, part 9545.0755 through 9545.0845. · The timeline to request reconsideration of the order is provided in Minnesota Statutes, section 142B.16, subdivision 2.
