5.1. Client Record

Client Record

The client status record is an unduplicated data record within a single agency. This means each record corresponds to one person who has received services at the time of the status update. Select the program/treatment type of service(s) the client was receiving at the time of the status update.

Example one: a client who received ARMHS and also received ACT services at the same time, from the same provider location is reported only once. ~ Both ARMHS and ACT services is reported together on one record.

Example two: a client who received ARMHS at one provider location and also received ACT at different provider location is reported by each provider location and will have two records. ~ One record from each provider location is reported.

Example three: a client who received ARMHS at one provider location and then changed to receiving ACT services at that same provider location is reported only once. ~ Change the Program/Treatment code to ACT services on the next client reported record.

Example four: a client who received Crisis service and then received mental health service at the same provider location is reported by each program type and will have two records. ~ One record from each program is reported.
