DATE ISSUED: June 16, 2023 DATE REISSUED: June 30, 2023 Miya Lindh Northstar Behavioral Health 1174 Western Ave. Fergus Falls, MN 56537 RE: Report No. 202303254 Northstar Behavioral Health License No. 1101848 Dear Miya Lindh: AMENDED CORRECTION ORDER An investigation of Northstar Behavioral Health was completed regarding report number 202303254, which alleged violation of Minnesota Statutes, section 626.557. This maltreatment allegation concerned neglect. The disposition of the investigation report is inconclusive. One violation was noted. VIOLATION AND CORRECTION ORDER The following violation of state and (or) federal law and rules was observed. Corrective action for the violation is required by Minnesota Statutes, section 245A.06, and is hereby ordered by the Commissioner of Human Services. Failure to correct the violation within the prescribed amount of time may result in fines and/or action against your license, as provided for in Minnesota Statutes, sections 245A.06 and 245A.07. Minnesota Statute 245G.09, subdivision 1, paragraph (a) requires the license holder (LH) to maintain a file for each client that contains the client’s current and accurate records. Citation: Minnesota Statute 245G.09, subdivision 1, paragraph (a) Violation: The license holder failed to maintain accurate client records. Through review of documentation and conducting staff person interviews, it was determined that the client record was inaccurate. The client record documented that the client was present at the facility when they were not. Corrective Action Required: The facility must ensure that all staff persons are accurately documenting in client records at all times.
