MHCP Enrolled Providers – Child and Teen Checkups (C&TC)/Early and Periodic Screening, Diagnosis and Treatment (EPSDT) Program

This training provides the standards/component requirements, and training of skills needed to perform various components including basic hearing and vision screening.
These online training programs are designed to provide knowledge and information needed to provide Minnesota children eligible for Child and Teen Checkups with quality health care.
These brochures are available in English and in the following languages: Hmong, Russian, Somali, Spanish and Vietnamese. To view/print these brochures in English or one of the other 10 languages.
These forms provide a checklist of age-specific screening components that inform families of what to expect at their child’s C&TC screening visits. There is a checklist for each of the recommended twenty visits between ages 1-2 weeks through 20 years.
