EW Customized Living Tools

DHS bulletin #11-25-05 includes 2011 Legislative Changes to Customized Living and 24 Hour Customized Living Services- Elderly Waiver: The customized living (CL) and 24 hour customized living (24CL) service rate limits, component rates, and individual authorized rates were reduced by 5% in the 2011 Legislative Session:

• "Instructions for Completion and Use of Customized Living Workbook (PDF) Complete instructions for Version 4 of Excel workbook (See Version Release Notes below for supplemental information on Versions 5 & 6). Counties, tribes and health plans using county networks or using DHS EW customized living tools or rates need to submit completed customized living workbooks to DHS within 30 days of customized service initiation or reauthorization. This document provides instruction for completing the workbook. See ‘Uploading your EW Customized Living Tools instructions’ below for directions on submitting workbooks to DHS :
