Primary Care Coordination program

The Primary Care Coordination (PCC) program is a way for Minnesota Health Care Program clients in fee-for-service with chronic and complex conditions to partner with their primary care provider to receive coordinated care in a new way.

  • • For patients, this means being able to see their doctor and other members of a care team more easily, better planning of care between visits, and the chance to participate in making the clinic experience work better.
  • • For primary care providers, this means delivering and coordinating care consistent with the PCC provider criteria and receiving reimbursement for this work.
  • PCC provides a "health care home" or "medical home" for eligible patients, and is an important milestone in efforts to make a health care home available to many more patients in Minnesota under the 2008 Health Reform Act.
