Health Care Homes (HCH) Eligible MHCP Organization and Individual Providers


Effective August 15, 2011, MHCP Fee for Service (FFS) HCH Tier e-Tool application became available in MN–ITS.

MN–ITS Provider Administrator (PA) or Provider Point Administrator (PPA) must first modify an individual’s MN–ITS user access to the HCH Tier e-Tool function. MHCP encourages administrators to allow HCH Tier e-Tool access only to staff (and their designees) assigned to treat patients directly and who have been trained to handle protected health information. Administrators, follow the steps below to give user access:

Select MN–ITS

Select User Administration

3.

Search for the individual user by name

4.

Select the individual in the User ID column

5.

Select the Health Care Home box in Applications/Forms/Lists on the PA/Users Profile

6.

Select Submit to add HCH access

After the HCH has been added to a user’s access it should be available within 45 minutes. The user may need to logoff and login to have the Health Care Homes application appear in the left navigation section of the MN–ITS Webpage.

The following providers are eligible to access HCH using their individual or the organizations NPI/UMPI once the user access has been modified:

  • • Ambulatory Surgery Center
  • • Audiologist
  • • Billing Entity for Physician Services
  • • Billing Entity for Rehabilitation
  • • Chemical Health
  • • Child and Teen Checkup Clinic
  • • Children’s Residential Services
  • • Chiropractor
  • • Clinical Nurse Specialist
  • • Community Mental Health Center
  • • Consolidated Provider Organizations
  • • Coordinated Care Delivery System
  • • Day Training & Habilitation Center
  • • Family Planning Agency
  • • Federally Qualified Health Center
  • • Home and Community Service
  • • Home Health Agency
  • • Hospice
  • • Hospital
  • • ICF/DD – Facility
  • • Independent Diagnostic Testing Facility
  • • Indian Health Facility
  • • Institution for Mental Disease
  • • Intensive Residential Treatment Services
  • • Licensed Professional Clinical Counselor
  • • Licensed Psychiatric Practitioner
  • • Marriage and Family Therapist
  • • Nurse Midwife
  • • Nurse Practitioner
  • • Nursing Facility
  • • Nursing Facility Control Organization
  • • Occupational Therapy
  • • Optometrist
  • • Pharmacy
  • • Optician
  • • Physician
  • • Podiatrist
  • • Pre-paid Managed Care Organization (MCO) Health Plan
  • • Psychologist

  • The following providers are eligible to access HCH as a designee of one of the above mentioned providers using their individual or the organizations NPI/UMPI once the user access has been modified:

  • • Health Care Coordinator
  • • Mental Health Rehabilitation Professional
  • • Personal Care Assistant
  • • Physician
  • • Physician Assistant
