  • Providers who would like to be eligible to provide, bill and be paid by MHCP for providing HCBS waiver services must:
  • • Find information for Minnesota Health Care Programs (MHCP) providers and MHCP Enrolled Providers – Home and Community Based (HCBS) Waiver and Alternative Care (AC) Programs Services
  • Enroll and continuously maintain qualifications to provide the waiver service
  • • Be selected by the service recipient, the recipient’s legal representative, if any, and the case manager or service coordinator of the lead agency, meaning the county, tribe or managed care organization (MCO)
  • • Have an approved service authorization (SA) to provide the service(s) for the recipient
