Request for Proposal Fiscal Year (FY) 2013
Due to a reduction in State funding for CS/SD Grants, the Department of Human Services is unable to fund applications in FY 12 and FY 13 for new construction, renovation or remodeling of existing buildings used to provide affordable services for older Minnesotans.

Purpose:
To expand and integrate home and community-based services for older adults that allow local communities to rebalance their long-term service delivery system, support people in their homes, expand the caregiver support and respite care network and promote independence
