The Minnesota Department of Health website has a Health Care Facility and Provider Database, with licensing information for long-term care facilities, such as nursing homes, boarding care homes and supervised living facilities, and for home care agencies. The site also has a database of complaint information for Minnesota’s health care providers. The Nursing Home Report Card has quality measures for nursing homes in Minnesota.

License Minnesota site has information on state agency licenses, permits

License Minnesota, a one-stop website with information on state agency licenses and permits, is available off the North Star website. North Star is the electronic entry point to Minnesota state government information and services. The License Minnesota website features licensing information on more than 500 licenses administered by more than 40 state agencies.
