This information is for MHCP-enrolled providers who serve MHCP members enrolled in the Minnesota Family Planning Program (MFPP). For non-MFPP family planning provider information, refer to the general MHCP family planning provider web page.
Confidentiality: Privacy protection applies to MFPP eligibility and benefits information. This information is private data on individuals, and cannot be released to anyone other than that individual without their informed consent, or unless the law otherwise allows. Minors may apply for MFPP without parental consent. Do not release information about a minor’s MFPP eligibility or benefits to a parent, guardian, or caretaker without the minor’s informed consent.
• MFPP changes effective December 2016
• On December 1, 2016, eligibility for MFPP is determined using modified adjusted gross income (MAGI) as reported on the member’s tax return. We have revised the MFPP application to reflect this policy change. • The application is not fillable at this time. Print the application, complete it and mail it to DHS at the address on the application. • We revised the MFPP Provider Training Guide (DHS-7421) (PDF) for the December 2016 changes.• MFPP changes effective January 2017 • The age limit was removed so anyone of any age can now apply for MFPP services. • Presumptive eligibility (PE) begins on the day a provider approves eligibility. PE ends on the date the full MFPP determination is completed or the last day of the month following the month it was approved, whichever is earlier. (Presumptive eligibility is allowed only one time in a 12-month period.) • An applicant may request MFPP eligibility retroactively for up to three months before the month in which he or she completes the MFPP application. However, retroactive eligibility cannot begin earlier than January 1, 2017. • MFPP covers nonemergency medical transportation (NEMT) services for dates of service on or after January 1, 2017. Refer to the NEMT section of the MHCP Provider Manual for more information about covered NEMT services and billing requirements.
• Certification as an MFPP provider: Certified MFPP providers must follow and adhere to the MFPP program-specific requirements defined in the MFPP Provider Training Guide (DHS-7421) (PDF). Failure to adhere to the requirements can result in disciplinary or monitory action • Before you determine PE you must verify eligibility in MN–ITS.
