Transition from PCA to CFSS

DHS is in the process of replacing personal care assistance (PCA) and the Consumer Support Grant (CSG) with Community First Services and Supports (CFSS). For more information about this transition, refer to CFSS Manual – Transition from PCA and CSG to CFSS.

Similarities

In both PCA and CFSS:

  • · The county/tribal nation enters X in each assessment field that has a corresponding “yes” (Y) in the person’s applicable assessment.
  • · Some fields are required.
  • Differences

    There are no differences between a PCA and CFSS service agreement (SA) on the AHC2 screen.

    Overview

    This page describes how to complete the AHC2 screen for both PCA and CFSS.

    The AHC2 screen contains assessment information MMIS uses to confirm the units/dollars the county/tribal nation enters on the ASA3 screen match the person’s eligibility.

    This screen is only used for a PCA or CFSS SA. For more information, refer to DSD MMIS Reference Guide – ASA3 screen overview.

    PCA and CFSS assessments fields

    The county/tribal nation enters X in each assessment field that has a corresponding “yes” (Y) in the person’s applicable assessment:

  • · MnCHOICES Assessment.
  • · PCA Assessment and Service Plan, DHS-3244 (PDF).
  • · CFSS Assessment, DHS-6893A.
  • The county/tribal nation leaves the assessment field blank when the corresponding box is checked “no” (N) in the person’s assessment.

    Referrals to other services field (REF COMP)

    The REF COMP field indicates if the assessor referred the person to other services.

    In both PCA and CFSS, the county/tribal nation enters the following in the REF COMP field:

  • · Y if the assessor referred the person to other services.
  • · NA if the assessor did not refer the person to other services.
  • If the county/tribal nation does not enter a valid value in the REF COMP field, MMIS posts an exception code the county/tribal nation must resolve.

    Shared services field (SHARED SERV)

    The SHARED SERV field indicates if the person can use shared services.

    In both PCA and CFSS, MMIS auto-populates the SHARED SERV field with Y. If the county/tribal nation edits this field, MMIS posts an exception code the county/tribal nation must resolve.

    Ventilator dependence field (EN)

    The EN field indicates if the person is ventilator dependent.

    In both PCA and CFSS, the county tribal nation enters the following in the EN field:

  • · Y (yes) when the person’s assessment results indicate the person is ventilator dependent.
  • · N (no) when the person is not ventilator dependent.
  • Provider owned or controlled housing field (PROV OWN/CTRL)

    The PROV OWN/CTRL field indicates if the person lives in housing owned or controlled by any PCA/CFSS provider they use (e.g., the person’s consultation services provider, PCA/CFSS provider agency or financial management services [FMS] provider). For more information, refer to CFSS Manual – PCA/CFSS provider-owned or -controlled housing.

    In the PROV OWN/CTRL field, the county/tribal nation enters:

  • · Y (yes) if one of the person’s PCA or CFSS providers owns or controls the person’s housing.
  • · N (no) if one of the person’s PCA or CFSS providers does not own or control the person’s housing.
  • If the county/tribal nation does not enter N, MMIS posts an exception code, and the county/tribal nation must deny the SA. For more information, refer to DSD MMIS Reference Guide – Notice of denial, reduction or termination of PCA/CFSS services.

    Navigation

    The county/tribal nation presses a transmit key to advance to the AHC3 screen.

    PCA and CFSS assessment field definitions

    TUBE FEEDING: Tube feeding.

    PRTR THYP: Parenteral/IV therapy.

    WOUNDS: Wounds.

    RESP ASSIST: Respiratory interventions.

    CATHETERS: Catheter insertion and maintenance.

    BOWEL PROG: Bowel program.

    NEUROLOGICAL: Neurological intervention.

    OTHER: Significantly increased need for hands-on assistance with six to eight activities of daily living (ADLs) due to a medical condition.

    LEVEL 1 BEHAVIOR: Level I behavior.
    Note: The LEVEL 1 BEHAVIOR field corresponds with the “presence of level I behavior” item and either a “yes” or “no” selection for “does the behavior happen more than four times a week” in the behaviors section of CFSS Assessment, DHS-6893A.

    COGNITIVE: Increased vulnerability due to cognitive deficits.
    Note: The COGNITIVE field corresponds with the “increased vulnerability due to cognitive deficits or social inappropriate behavior” item and requires a “yes” selection for “does the behavior happen more than four times a week” in the behaviors section of CFSS Assessment, DHS-6893A.

    RESISTIVE: Resistive to care, including verbal agression.
    Note: The RESISTIVE field corresponds with the “resistant to care with verbal aggression” item and requires a “yes” selection for “does the behavior happen more than four times a week” in the behaviors section of CFSS Assessment, DHS-6893A.

    AGGRESSION: Level 1 behavior at least four times per week.
    Note: The AGGRESSION field corresponds with the “presence of level 1 behavior” item and requires a “yes” selection for “does the behavior happen more than four times a week” in the behaviors section of CFSS Assessment, DHS-6893A.

    DRESSING: Dressing.

    GROOMING: Grooming/hygiene.

    BATHING: Bathing.

    EATING: Eating.

    TRANSFERS: Transfers.

    MOBILITY: Mobility.

    POSITIONING: Positioning.

    TOILETING: Toileting.

    Removed fields

    DHS removed the following fields from MMIS in September 2024:

  • · FSG.
  • · ASST/MED.
  • · TREAT/MNT.
  • · SEIZURES.
  • · SELF INJURIES.
  • · HURT OTHERS.
  • · HABITS.
  • · LEVEL3BEH.
  • · I/SADL.
  • · APPL/EQUIP.
