Acceptance of Appointment as Personal Representative and Oath by Individual (DHS-8348)

Follow the instructions for (DHS-8348) (PDF) to accept your appointment as personal representative of an estate.

1. Fill in the header.

· Enter your county name in the top left.

· Enter the deceased person’s name on the left.

· Enter the court district number in the top right. If you are unsure of your district, look up your court district.

· Since you are opening a new court file, you do not need to enter a file number.

2. Fill in the body.

· Place your name (or the name of the person who will be the personal representative for the case) in the first blank.

· Enter the county office’s address in the second blank.

3. Put your county attorney’s information in the lower left box.

4. Date and sign the document.
