Certificate of Representation (DHS-8348B)

Follow the instructions for (DHS-8348B) (PDF) to provide the court information on what attorney is representing the county.

1. Fill in the header.

· Enter your county name in the top left.

· Enter the deceased person’s name on the left.

· Enter the court district number in the top right. If you are unsure of your district, look up the court district.

· Since you are opening a new court file, you do not need to enter a file number.

2. Fill in the body.

· Enter the name of the person who is the petitioner and personal representative in the first blank.

· Enter the county attorney’s information. This is the information of the county attorney actively representing the county in this case.

3. Put the county name in the lower right blank.

4. The county attorney actively representing the county in this case dates and signs the document.
