Background | Screening teams determine the level of care needed by the person and identify the least restrictive services and their cost effectiveness. For people with a diagnosis of developmental disability or a related condition, screening teams convene to evaluate the level of care needed by the person when the assessment indicates that the person: · Is at risk of placement in an intermediate care facility for persons with developmental disabilities· Is at risk of placement in a nursing facility or· Requests services in the areas of residential, training and habilitation, nursing facility or family support. |
Screening team responsibilities | Screening teams are responsible to review: · Current Individual Service Plans · Other data related to the person’s eligibility and need for services, as determined necessary by the screening team · Previously completed DD Screening Document fields and adjust as needed, given feedback from screening team members· Results of diagnostic evaluations and assessments of the person's needs for services and supports.Screening teams are responsible to identify: · Cost implications of recommendations· Level of services appropriate to maintain the person in the most normal and least restrictive setting that is consistent with the person’s treatment needs· Other non-institutional public assistance or social service supports that may prevent or delay long-term residential placement.Screening teams are responsible to assess whether people need long-term residential care and make recommendations regarding placement and payment for: · Community residential placement· Home and community-based service alternative to community residential placement or regional treatment center placement· RTC placement · Social service or public assistance support, or both, to maintain a person in the person’s own home or other pace of residence· Training and habilitation service, vocational, rehabilitation and employment training activities.Screening teams are responsible to evaluate the availability, location, and quality of the services listed above, including the impact of placement alternatives on the person’s ability to maintain or improve existing patterns of contact and involvement with parents and other family members; as well as other natural support people and systems. Screening teams are responsible to: · Inform the person and their legal representative, if any, that they may appeal decisions to the Commissioner· Make recommendations to a court as needed to assist the court in making decisions regarding commitment. |
Screening team composition | Screening teams consist of: · Person being screened· Person’s parent(s), if the person is a minor· Person’s legal representative (if any)· Person’s case manager/service coordinator · Qualified developmental disabilities professional (QDDP) · Registered nurse, if the person has overriding health care needs, who must serve as the QDDP or case manager/service coordinator · Person’s advocate (if any) upon the request of the person or their legal representative.All screening team members must be physically present. If case managers/service coordinators are qualified QDDPs, they may also act as QDDP during screenings. Case managers/service coordinators may not serve as both the case manager/service coordinator and the legal representative when people are under public guardianship. Other people may be present during the first part of the screenings to provide information about people being screened at the request of: · Case managers/service coordinators· People being screened or their legal representatives.This might include service providers or advocates. No member of the screening team shall have any direct or indirect service interest. |
Service provider input | Solicit service provider input regarding assessment information prior to the screenings when possible. The presence of service providers can impede the informed choice process. However, if service providers are invited to attend the screening they are: · Limited to providing consumer assessment information and · Not allowed to participate in the decision-making process.Service providers should leave screening team meetings: · After the assessment section is completed and · Prior to any discussion of planned services. |
