Relocation and transition assistance

This section of the manual includes information about how to authorize:

  • · Relocation service coordination targeted case management (RCS-TCM).
  • · Alternative Care (AC) conversion case management.
  • · Moving Home Minnesota (MHM) services.
  • Relocation and transition assistance under managed care

    The managed care organization (MCO) is responsible for service planning and payment of relocation assistance while a person is enrolled in Minnesota Senior Health Options (MSHO) or Minnesota Senior Care Plus (MSC+).

    Transitional services for waiver programs

    Transitional services support people to relocate and transition to community-based living successfully. For information about covered and non-covered transitional services, refer to:

  • · CBSM Transitional services Brain Injury (BI), Community Alternative Care (CAC), Community Access for Disability Inclusion (CADI) and Developmental Disabilities (DD) waivers.
  • · CBSM Transitional services Alternative Care (AC) and Elderly Waiver (EW).
  • You authorize these services on a service agreement. For information about service agreements, refer to MMIS LTSS Manual – Work with service agreements.
