Moving Home Minnesota (MHM) services

MHM transitional services provide up to 180 days of service to qualified people living in a qualified institution who have resided in one or more qualified institutions for at least 60 consecutive days. DHS can grant extensions to the 180-day limit on a case-by-case basis. For more information and policy about MHM, refer to MHM Program Manual.

Instructions to authorize MHM in the long-term care (LTC) screening document

The ALT4 screen in the LTC screening document in MMIS has a field called “MHM.” DHS removed this question from the MnCHOICES LTC screening document. You can leave this field blank in MMIS. It does not affect the person’s eligibility for MHM.

MHM and relocation service coordination targeted case management (RSC-TCM)

A person cannot have RSC-TCM and MHM at the same time. If a provider submits RSC-TCM claims for dates of service after a person begins MHM, the RSC-TCM claims will deny. If a provider submits MHM claims with dates of service in the same month as RSC-TCM, MHM claims will deny if RSC-TCM claims were paid first.

To request reprocessing of MHM claims that are overlapping and paid in the same month as RSC-TCM, contact DHS using the MHM Communication Form, DHS-6759H and select the “Denied Claim(s)” communication reason.

MHM and Brain Injury (BI), Community Alternative Care (CAC) and Community Access for Disability Inclusion (CADI) and Elderly Waiver (EW),

People who receive BI, CAC, CADI and EW services may also receive the following MHM services when authorized on their waiver service agreement:

  • · Community education and integration costs (T2025 U6).
  • · Comprehensive community support services (H2015 U6).
  • · Home care training – family and non-family (S5116 U6).
  • · Membership fees (S9970 U6 U5).
  • · Pantry stocking (S9977 U6).
  • · Records and fees (T5999 U6, S9981 U6).
  • · Transition integration fund (T1999 U6, K0899 U6, S5199 U6).
  • The costs of these services do not apply toward the EW budget cap.

    When adding MHM services to a waiver service agreement, you must add reason code 586 to the line item reason code field to avoid exception code (i.e., edit) 166: “Requires reason code 586 to be included on lines with the U6 modifier indicating MHM service.”

    Before you add services to the waiver service agreement, the person must be determined eligible for MHM by DHS. For eligibility requirements, refer to MHM Manual – Eligibility.

    MHM and managed care organizations (MCOs)

    The MCO pays for transitional services for a person age 65 or older who is enrolled in Minnesota Health Options Services (MSHO) or Minnesota Senior Care Plus (MSC+). The MCO must coordinate or provide the person with MHM transition coordination if they are enrolled in MSHO or MSC+.

    A person age 65 or older must enroll in EW to access MHM services, unless they meet an exception and return to a disability waiver upon their return to the community. The MCO is responsible to authorize MHM transitional services not covered by the waiver.

    If a person enrolled in MSHO/MSC+ and EW moves to the community, the MCO is responsible to authorize both MHM community services and EW services.

    If a person returns to a disability waiver upon their return to the community, the county/Tribal Nation is responsible to authorize MHM community services with the person’s disability waiver services.
