2026 MMIS rate remediation plans

Note: On March 31, 2026, DHS updated the title of this page from 2026 Disability Waiver Rate System (DWRS) rate remediation to Medicaid Management Information System (MMIS) rate remediation plans because lead agencies must complete rate remediation using MMIS, not the DWRS rate calculation tool in MnCHOICES.

Lead agencies and providers must plan to remediate rates in MMIS for 15 DWRS framework services with start dates of Jan. 1, 2026, or later.

Centers for Medicare & Medicaid Services (CMS) approval

On March 25, 2026, DHS received CMS approval to use the 2026 component values included in both the Feb. 6, 2026, and March 6, 2026, MnCHOICES releases. For more information, refer to the March 31, 2026, eList announcement.

Affected services

The following DWRS framework services will require rate remediation:

  • · Adult day services – 15 minute.
  • · Adult day services – daily.
  • · Day support services.
  • · Prevocational services – 15 minute.
  • · Prevocational services – daily.
  • · Employment services (i.e., employment support, employment exploration, employment development).
  • · Individualized home supports (IHS) with family training – 15 minute.
  • · IHS with training – 15 minute.
  • · IHS with training – daily.
  • · IHS without training.
  • · Night supervision.
  • · Positive supports (i.e., professional, analyst and specialist).
  • · Family residential services (FRS), including FRS, adult daily; FRS, child daily and FRS, adult, life sharing daily.
  • · Community residential services.
  • · Integrated community supports (ICS).
  • Note: Employment support services is the only service with a slightly lower remediation rate.

    DHS will not require rate remediation for:

  • · Adult day bath.
  • · Customized living (CL).
  • · 24-hour CL.
  • · Any of the above 15 framework services that have a DHS-approved exception rate.
  • · FRS rates calculated using the DHS-provided amended FRS framework before April 1, 2026.
  • Lead agency actions

    Lead agencies must:

  • · Immediately: Disregard the instructions DHS emailed to DWRS rate mentors on March 25, 2026.
  • · Through March 31, 2026: Continue to use the amended FRS framework for starting dates between Jan. 1, 2026, and March 31, 2026.
  • · Effective April 1, 2026: Implement the FRS tiered rates at the person’s next waiver renewal and/or if the person has a change in service (i.e., change in condition, change in service provider). Refer to DHS – Family residential, life sharing tiered rates FAQ.

    MMIS rate remediation plans

    In May 2026, DHS will send lead agencies remediation plans with detailed instructions.

    The remediation plan will include:

  • · Affected MMIS service lines.
  • · Current rates in MMIS.
  • · Corrected rates to authorize in MMIS starting June 1, 2026.
  • · The difference between the billed unit rate and the corrected unit rate.
  • · A template for lead agencies to complete and give to providers to document remediation efforts.
  • Claim voids information for providers

    Providers will have up to a year from the service date to complete claim voids for rates they want to rebill at the correct rate. DHS will not require forced claim voids for under-billed rates.
