Definitions | MHM home and community-based services (HCBS) coordinator: Professional who helps with intake and confirms a person’s eligibility to participate in MHM. SUD services: Services that include assessment of needs, treatment planning and interventions to address a person’s needs as a result of substance use. 1115 SUD System Reform Demonstration: A federal waiver that implements the American Society of Addiction Medicine (ASAM) criteria and specific levels of care. ASAM criteria: The clinical guidelines for purposes of assessment, treatment, placement and transfer or discharge of people with SUD. Licensed alcohol and drug counselor (LADC): A person who holds a valid license issued under Minn. Stat. §148F.01 to provide alcohol and drug counseling. Alcohol and drug counselor (ADC): A person who holds a certification from the Upper Midwest Indian Council on Addictive Disorders (UMICAD) within American Indian SUD programs. Lead agency: County, tribal nation or managed care organization (MCO). | ||

Qualified institutions for SUD | A qualified institution for SUD must meet all of the following requirements: | ||
Enrollment | To enroll a person in MHM, anyone can complete MHM Intake Form, DHS 5032. The person completing the intake form can submit it online via the form submission button. | ||
Eligibility | The LADC or ADC must use the person’s comprehensive assessment (as described in Minn. Stat. §245G.05, subd. 1) to support the completion of Professional Statement of Need (PSN), DHS-7122 (PDF). The PSN establishes the person’s eligibility for MHM and meets the level of care requirement. The LADC or ADC can submit the PSN: Moving Home Minnesota P.O. Box 64250 St. Paul, Minnesota 55164-0250 For additional information about the PSN, refer to PSN Guidance for Qualified Professionals, DHS-7122 (PDF). The MHM Eligibility and Enrollment team contacts the person and the LADC or ADC within 30 days to let them know the person’s eligibility status. After receiving approval from MHM Eligibility and Enrollment, the LADC or ADC and the person work together to choose a transition coordinator. Then, the LADC or ADC completes and sends MHM Transition Coordination Services Referral Form, DHS-6759N to the transition services provider to start the person’s transition coordination services. | ||
