Specific Instructions - Social Services, Assessment, and Other
Documentation and Verification for these activities can vary, and can include the following:
Assessment: professional mental health or chemical dependency assessments, ES Counselor’s assessments and/or case notes that assess participants for (including but not limited to): chemical health, mental health, physical health, child behavior, personal skills, childcare, dependent care, transportation, legal, safe living environment, housing, financial, education, and social support (Employability Measure). If this information is already in Workforce One, additional documentation in paper file is not necessary.
Other: Medical Opinion Form. Correspondence and other statements from qualified medical providers.
Social Services: Treatment plans, correspondence from health care, chemical dependency, and mental health professionals, counseling, meeting with advocates, and child protection (signatures of professionals are recommended to reduce complication).
Homeschooling, distance, and hybrid learning is not considered a core or non-core activity. It can be tracked as “other” activity and be included in the Employment Plan if childcare or other support services are needed for the other children. The hours spent in this activity will not count towards the WPR. Non-FSS participants or participants without good cause will still need to meet the minimum hourly requirements for participation hours.
MFIP participants may require social, cultural, medical, or other services to address circumstances that prevent them from getting or holding a job.
Use “Other” activity type, if the activity a participant needs in their plan to address their specific challenges is not listed under any other activity, including culturally specific activities.
