Transition from CSG to CFSS

DHS is in the process of replacing personal care assistance (PCA) and the Consumer Support Grant (CSG) with Community First Services and Supports (CFSS). For more information about this transition, refer to CFSS Manual – Transition from PCA and CSG to CFSS.

DHS must update the Medicaid Management Information System (MMIS) to allow counties/tribal nations to enter a CFSS service agreement (SA). Some fields have minor changes. For more information, refer to DSD MMIS Reference Guide – MMIS transition from PCA and CSG to CFSS.

During the transition from CSG to CFSS, the county/tribal nation follows the instructions below to enter a new SA in MMIS:

  • · Conducts the assessment.
  • · Enters three months of CSG services with the CSG procedure code (T2025).
  • · Enters a line for the county/tribal nation administrative fee with the CSG procedure code (T2025).
  • · Enters a line for consultation services. For more information, refer to DSD MMIS Reference Guide – ASA3 screen for consultation services (T1023).
  • CSG services

    Lead agency staff enter type B service agreements into MMIS to document the results of PCA assessments and any services authorized for CSG.

    Use the results on the PCA Assessment and Service Plan (DHS-3244) to complete the PCA assessment fields in MMIS.

    MMIS denies claims for payment of services that do not have current authorization.

    Service agreement letters

    MMIS generates service agreement letters in an overnight process. DHS sends out the letters to the recipient and the provider or providers listed on service agreement lines. Service agreement letters communicate results of assessments, service dates and any services authorized.

    Service agreement letters provide legal notice to recipients and providers of a reduction, termination or denial of CSG services. Letters include text associated with any reason codes entered.

    Type B SA for CSG with combined PCA and HCN or HHA services

    Begin at the AKEY service agreement key screen to review prior authorization history or initiate a new service agreement.

    Assessments may determine the grant amount for PCA, home care nursing and home health aide services.

    HCN services require HCN assessment, plan of care and request for services by home health agency sent to DHS and entry of a Type B SA for CSG into MMIS by worker.

    HHA services require a plan of care and request for services by home health agency sent to DHS and entry of a Type B SA for CSG into MMIS by worker.

    DHS combines HCN or HHA request with the Type B SA for CSG and sends the supporting documentation to contractors for review of medical necessity and determination of home care rating.

    Cashing out PCA, HCN, HHA

    Enter new service agreement for CSG using the PCA assessment results on the current PCA service agreement. DHS adjusts CSG service agreement to include HCN and HHA services and closes current service agreement.
