DD Screening Document Codebook
The Developmental Disabilities (DD) Screening Document Codebook is a reference tool for lead agency workers responsible to complete DD screening documents. The completed and authorized DD screening document for people with developmental disabilities or related conditions records compliance with Minnesota's federally approved waiver plan and Medicaid state plan.
Reimbursement for Medical Assistance services (e.g., intermediate care facilities for persons with developmental disabilities [ICFs/DD], waiver services and nursing facility services) cannot occur without DHS’ authorization of the DD screening document.
