Work with long-term care (LTC) screening documents

This section of the manual provides instructions for data entry in the LTC screening document. The LTC screening document is located in the prior authorization subsystem. For overview information about that subsystem, refer to MMIS LTSS Manual – MMIS subsystems.

The LTC screening document captures information about:

  • · Activities related to long-term care consultation (LTCC).
  • · Screenings and assessments.
  • · Eligibility determination and program management for Alternative Care (AC), Brain Injury (BI) Waiver, Community Access for Disability Inclusion (CADI) Waiver, Community Alternative Care (CAC) Waiver, Elderly Waiver (EW) and Essential Community Supports (ECS).
  • Data in the LTC screening document supports the authorization of services and payment to providers for these programs. For information about service authorization, refer to MMIS LTSS Manual – Work with service agreements.

    Note: The LTC screening document does not capture information about the Developmental Disabilities (DD) Waiver. For information about that waiver, refer to the DD Screening Document Codebook.
