Program Integrity

Counties have the most direct access to those needing and receiving services from state-administered programs. County workers serve as trusted partners with both program recipients and the Minnesota Department of Human Services (DHS) and Department of Children, Youth, and Families (DCYF).

Report fraud, waste or abuse of public funds

When program recipients share any information with county or Tribal human services workers around issues in services that could be related to fraud, waste or abuse of public funds, please report it immediately. Use the Fraud reporting tip form to report suspected fraud at either agency.

Continuity of care planning for potential service disruptions

The Minnesota Department of Human Services hosts a weekly planning meeting for lead agency staff to discuss continuity of care for people whose providers have been disenrolled as part of Minnesota Revalidate 2026. The meeting also covers other program integrity impacts to service recipients, shares updates on current processes and challenges, and discusses strategies for ensuring uninterrupted care.

Meetings are held virtually on Fridays from 9 to 10 a.m., via Microsoft Teams. Registration is required – to learn more about the meetings and register for upcoming meeting dates, see the AASD and DSD eList announcement.

  • Minnesota Revalidate. Minnesota Revalidate 2026 is an urgent statewide Medicaid provider revalidation effort required by the Centers for Medicare & Medicaid (CMS). All providers of services designated as high risk in Governor Walz’s Oct. 29 announcement received a revalidation determination as of May 31, 2026. Many providers are now appealing their disenrollment determinations. See the Revalidation process FAQ for details.
