Purpose: To announce MnCHOICES updates that affect the developmental disabilities screening document and MMIS entry process
DHS made the following changes:
Because of these changes, DHS no longer requires lead agency staff to enter the long-term care (LTC) screening document into MMIS for people who use DD programs or services. DHS will update and publish MnCHOICES forms referencing this process in the near future.
When MnCHOICES launched, DHS required lead agency staff to enter both a developmental disabilities (DD) screening document and a long-term care (LTC) screening document into MMIS. This requirement allowed staff to track when they last completed a MnCHOICES assessment.
Effective immediately, lead agency staff should use code 60 (MnCHOICES) in field 41 (current services) on the DD screening document to capture if a MnCHOICES assessment was completed.
Lead agency staff should stop entering the LTC screening document from MnCHOICES into MMIS for people who use DD programs or services.
