Referral for PCA reassessment

Policy

A PCA provider agency must send the Referral for Reassessment for PCA Services (DHS–3244P [PDF]) to the lead agency 60 days before the end of the service agreement.

Assessor responsibilities

Lead agency assessors must:

1. Accept referrals for PCA reassessments from PCA agencies.

2. Schedule and complete an assessment for PCA services before the end of the existing service agreement.

Lead agencies may use Infopak report 2482 (service agreements ending in 60 days) to track the completion of reassessments.

PCA provider agency responsibilities

PCA provider agencies must:

1. Complete the referral and fax it to the appropriate assessing lead agency.
2. File the original referral form in the person’s file.
3. Confirm the assessment has been scheduled and completed before the end of the service agreement.

DHS responsibilities

DHS must:

1. Send notification letters to the participant and current provider 60 days before the end of the service agreement.
2. Generate Infopak reports and service agreement reports to lead agencies.
