Referral for PCA reassessment

Policy

A PCA Provider Agency must send the Referral for PCA Services, DHS–3244P, (PDF) to the lead agency 60 days before the end of the service agreement.

Assessor responsibilities

Lead agency assessors are responsible to:

1. Access the InfoPac 60-day report to get list of PCA reassessments due.
2. Accept the referrals from the PCA provider agencies.
3. Schedule and complete the assessment for PCA services before the end of an existing service agreement.

PCA provider agency responsibilities

PCA provider agencies are responsible to:

1. Complete the referral and fax to the appropriate assessing lead agency.
2. File the original in the consumer’s file.
3. Confirm the assessment has been scheduled and completed before the end of the service agreement.

DHS responsibilities

DHS is responsible to:

1. Send letters to the consumer and current provider 60 days before the end of the service agreement.
2. Generate and make InfoPac reports available for a county resource.
