An investigation of REM North Star Inc was completed regarding report number 202510543 which alleged violation of Minnesota Statutes, section 626.557, and Minnesota Statutes, Chapter 245D. VIOLATIONS AND CORRECTION ORDERS

The following violation of state and (or) federal laws and rules were observed. Corrective action for the violation is required by Minnesota Statutes, section 245A.06, and is hereby ordered by the Commissioner of Human Services. Failure to correct the violation within the prescribed amount of time may result in fines and/or action against your license, as provided for in Minnesota Statutes, sections 245A.06 and 245A.07.

Citation: Minnesota Statutes, section 245A.04, subdivision 13, paragraphs (a), (b), and (c).

Violation: The license holder did not ensure that a person served by the program retained the use and availability of personal funds; did not ensure separation of funds of a person served by the program from funds of a program staff; and did not immediately document receipt and disbursement of a person's funds or other property at the time of receipt or disbursement.

A consumers (C’s) guardian sent the C’s monthly personal needs money to a staff person’s (P) bank account via Zelle. The P was to disperse the money to the VA but did not consistently track the VA’s disbursements and/or expenditures.

Corrective Action Ordered: On an ongoing basis, you must maintain compliance as required in this subdivision.
