Additional Sources of Revenue

7 pie pieces divided into MN Public Programs, Government Grants, Private Grants and Philanthropy, Community and Organizational Contributors, Individual Contributors, Fundraisers, and Other Sources with MN Public Programs hightlighted Local counties, health plans, and tribes are responsible for an assessment of a person’s needs to determine whether a person is eligible for services.   Financial eligibility must also be determined by the local county financial unit.  If eligible, each person is assigned a Case Manager (or Care Coordinator).  The Case Manager is responsible for setting up a service plan, arranging services, and monitoring services.
